摘  要:目的探讨宫颈鳞状细胞癌(SCC)组织Smad4表达与临床病理特征的关系。方法采用免疫组织化学SP法检测28例正常宫颈上皮(NCE)、36例宫颈上皮内瘤变(CIN)和68例SCC组织中Smad4的表达情况,检测Ki-67增殖指数(PI)和CD34微血管密度(MVD)。结果Smad4在NCE,CIN及SCC组的阳性表达率分别为75.00%,41.67%和45.59%。从NCE到CIN及SCC,Smad4阳性表达率显著降低(P<0.05)。在SCC组,随Smad4表达降低,盆腔淋巴结转移率显著升高(χ2=4.446,P=0.035);随Smad4表达增强,PI和MVD均有升高趋势,均未达统计学意义(P>0.05)。SCC组Smad4表达与年龄、FIGO分期、组织学分级、浸润深度、脉管浸润与否及预后无显著相关性(P>0.05)。结论Smad4蛋白表达降低可能参与SCC的发生和转移过程。Objective To investigate expression of Smad4 and their correlation with clinic- pathologic characteristics in cervical squamous cell carcinoma. Methods The expression of Smad4, proliferation index(PI) of cancer cells labeled by Ki-67 and microvessel density(MVD) labeled by CD34 in 28 cases of normal cervical epithelium(NCE), 36 cases of cervical intraepithelial neoplasm(CIN) and 68 cases of squamous carcinoma of cervix(SCC) were detected by immunohistochemistry SP method. Results The positive rates of expression of Smad4 in NCE, CIN and SCC group were 75.00%, 41.67% and 45. 59%, respectively. The positive expression of Smad4 decreased remarkably from a sequence of NCE to CIN and SCC(P〈0.05). The rate of pelvic lymph node metastasis in SCC was significantly increased following Smad4 lower expression(X^2 =4. 446, P=0.035). The PI and MVD in SCC were increased following Smad4 higher expression, but it was not significant(P〉0.05). The expression of Smad4 in SCC was not correlated with patients age, FIGO staging, histological grading, stroma involvement, intravascular infiltration or prognosis(P〉0.05). Conclusion The loss expression of Smad4 protein may participate in pathogenesis and metastasis in cervical squamous cell carcinoma.
