摘  要:目的观察飞秒激光制作角膜瓣的波前像差引导的LASIK手术早期治疗效果,并与常规波前像差引导的LASIK手术比较。方法在波前像差引导的个体化角膜切削前,分别使用飞秒激光机和Moria M2微型角膜板层刀为各30例(飞秒组和机械组各60只眼)近视患者制作同厚度角膜瓣,手术前后观察裸眼视力、眼波前像差、对比敏感度,并比较两组患者临床效果。结果术后3个月两组裸眼视力均提高,有效性和安全性指数:飞秒组:1.19;1.20。机械组:1.16;1.15。术后两组等效球镜度数变化相似。两组术后3个月,飞秒组球差增加小于机械组。飞秒组最高空间频率(18c/deg)对比敏感度比术前提高。机械组相同频率对比敏感度术后1月下降,术后3个月恢复至术前水平,结论与使用Moria M2微型角膜板层刀制作角膜瓣的波前像差引导的个体化LASIK手术比较,使用飞秒激光制作角膜瓣的波前像差引导的个体化LASIK手术后早期,裸眼视力相似,球差增加较少,屈光状态更稳定,高空间频率的对比敏感度增加更多,显示出更好地治疗效果。Objective Evaluate the effect of customized laser in situ keratomileusis surgery performed with femtosecond laser. Methods In this prospective, nonrandomized, masked study, 120 eyes undergoing customized LASIK were included in this study: 60 eyes with the Moria M2 (M2) microkeratome and 60 eyes with the IntraLase femtosecond laser (IL) . BSCVA, refractive errors, wavefronterrors and contrast sensitivity were evaluated before surgery, 1 month and 3 months after surgery. Results All the patients in both groups showedanincreaseinvisualacuityafterLASIK (P〈0.001).3 monthsaftersurgerycontrastsensitivityfimctions. (CSFs) of 18 cpd in IL group was higher than that before surgery. The increase of coma ofwavefront error in IL group was less than that in M2 group. There was no significant difference between two groups in refractive errors, visual acuity 3 months after surgery. Conclusion wavefront guided LASIKperformed with femtosecond laser may result in better visual function than that performed with Moria M2 microkerotomy in some contrast sensitivity, comawavefront error and stabilization of refractive errors.
