摘 要:目的:研究葡萄糖转运蛋白1(Glut-1)、C-erbB-2在子宫内膜增生性病变和子宫内膜腺癌中的表达特点及其诊断价值。方法:应用免疫组织化学SP法检测20例正常增生期子宫内膜、23例单纯性子宫内膜增生、21例复杂性子宫内膜增生、43例非典型性子宫内膜增生、25例子宫内膜腺癌中Glut-1、C-erbB-2的表达。结果:Glut-1、C-erbB-2在各实验组中的阳性表达率增生期子宫内膜组均为0,单纯性增生组分别为0和4.5%,复杂性增生组为4.8%、4.8%,低级别非典型增生组为20.0%、30.0%,高级别非典型增生组为52.2%、60.9%,腺癌组为88.0%、92.0%;Glut-1、C-erbB-2在各实验组间差异均有显著性(P<0.01),在高级别非典型增生组与子宫内膜腺癌组间差异均有显著性(P<0.01)。C-erbB-2与Glut-1在各组的表达呈显著正相关关系(P<0.01)。结论:Glut-1、C-erbB-2可能在子宫内膜癌的发生发展中共同发挥重要作用,二者联合检测对子宫内膜腺癌及癌前病变的鉴别诊断有较高的诊断价值。Objective: To study immunohistochemical expression of Glut-1 and C-erbB-2 in benign, hy- perplastic and malignant endometrial epithelia. Methods: Immunohistochemical staining S-P method was used to detect the expression of Glut-1 and C-erbB-2 in 20 cases of normal prolifera-tive endometrium,23 cases of simple endometrial hyperplasia, 21 cases of complex endometrial hyperplasia, 43 cases of atypical endometrial hyperplasia and 25 cases of endometrial adenocarci-noma. Results: The expression of Glut-1 and C-erbB-2 had not been detected in normal prolifera- tive endometrium, while the expression rate was 0 and 4.5% in simple endometrial hyperplasia, 4.8% and 4.8% in complex endometrial hyperplasia, 20.0% and 30.0% in low-grade atypical endometrial hyperplasia, 52.2% and 60.9% in high-grade atypical endometrial hyperplasia, 88.0% and 92.0% in endometrial adenocarcinoma. There were significant differences among benign, hyperplastic and malignant endometrial epithelia, the difference between endometrial adenocarcinoma and high-grade atypical endometrial hyperplasia were also statistically significant. Positive correlation was also found between C-erbB-2 and Glut-1 expression (P〈0.01).Conclusion. Abnormal expression of Glut-1 and C-erbB-2 may contribute to the pathogenesis and development of the endometrical carcinoma. The combined use of Glut-1 and C-erbB-2 is valuable in distinguishing endometrial adenocarcinoma from endometrial hyperplastic lesions.
