摘 要:目的:观察普罗布考对脑梗死患者急性期血清氧化型低密度脂蛋白自身抗体(oxLDL-Ab)和基质金属蛋白酶9(MMP-9)水平的影响。方法:采用自身对照和组间对照,将62例急性期脑梗死患者分为两组,其中普罗布考组(32例),常规组(30例)。治疗2周后,用酶联免疫吸附法和酶谱法分别测定治疗前后血清oxLDL-Ab及MMP-9水平的变化。以神经功能评分为标准,分别观察治疗7 d和14 d后两组的疗效。结果:①普罗布考组治疗2周后,血清oxLDL-Ab及MMP-9水平均较治疗前明显降低(均P<0.05),神经功能明显改善(P<0.05)。②治疗2周后,普罗布考组血清oxLDL-Ab及MMP-9水平均较常规组明显降低(均P<0.05),神经功能明显改善(P<0.05)。③普罗布考组治疗前后,血清oxLDL-Ab及MMP-9水平降低无相关性(P>0.05)。结论:普罗布考能降低脑梗死患者急性期血清oxLDL-Ab及MMP-9水平,可能通过稳定动脉粥样硬化斑块,抑制了血-脑屏障的早期开放和脑水肿,从而减轻缺血性脑损伤,防止脑梗死出血转化和再发,并同时提高了患者的近期疗效。Objective: To observe the effect of probucol on the levels of anti-oxidized LDL auto-antibod- ies (oxLDL-Ab) and matrix metalloproteinase-9(MMP-9) in acute stage of cerebral infarction. Methods: By random and single blind contrast methods, 62 patients with cerebral infarction in acute stage were divided into probucol group (n=32)and convention treatment group(n=30). With the methods of ELISA and SDS-Page zymograph,the levels of oxLDL-Ab and MMP-9 in serum were measured before and after therapy. At 7 d and 14 d, curative effects were evaluated by standard scale of clinical neurological deficit. Results:①After two weeks of follow-up,the levels of oxLDL-Ab and MMP-9 in serum were obviously decreased (P〈0.05) and the neurological function was improved (P〈0.05) in the probucol group.②The levels of oxLDL-Ab and MMP- 9 in serum were obviously decreased (P〈0.05) and the neurological function was improved (P〈0.05) in the probucol group compared with the convention treatment group after two weeks'therapy. ③The decreasing of MMP-9 was not depended on the decreasing of oxLDL-Ab before and after therapy in the probucol group(P〉0.05). Conclusion. Probucol can reduce the levels of oxLDL-Ab and MMP-9 in acute stage of cerebral infarction, probably, stabilize atherosclerotic plaque and inhibit the disruption of blood-brain barrier in early stage and cerebral edema to relieve the ischemic brain injury and prevent hemorrhagic transformation and recurrence of cerebral in- farction.
