摘　　要：目的探讨原发性胃恶性淋巴瘤的诊断治疗,以提高诊断率和选择合适的治疗方案。方法回顾我院66例胃恶性淋巴瘤的临床资料,分析其误诊原因、治疗方法及5年生存率。结果胃原发性淋巴瘤误诊率高,治疗应在切除原发灶的基础上结合放疗及化疗,其5年生存率较高。结论影响PGL预后的关键因素是临床分期(肿瘤大小、部位、浸润度及淋巴结转移范围等)以及手术术式,治疗方案的设计。应争取早期发现、早期诊断、早期治疗,治疗上采取以手术为主,加放疗、化疗的综合治疗方案,近年来配合生物治疗可望进一步改善生存期。Objective To study the diagnosis and treatment of pimary gastric lymphoma. Methods 66 primary gastric lymphoma were studied according to clinical data, reason of misdiagnosis, treatment and 5 years survival rate. Results Primary gaswic lymphoma misdiagnosis rate was high, operation combined with radiotherapy and chemotherapy was the main treatment method, 5 years survival rate was higher than other gastric tumors. Conclusion The key factors to influence the prognosis of PGL are clinical term（include tumor size,location,deep of infiltration and metastasis Scope of lymph node）,operation manner and clinical therapy design. We should do early discovery,diagnosis and treatment, and the treatment is combined therapy which operation to assist radiotherapy and chemotherapy. Reently use bioremediation hope to improve survival time.
