摘 要:目的探讨基质金属蛋白酶-14(matrix metalloproteinase-14,MMP-14)和细胞外基质金属蛋白酶诱导因子(extra-cellular matrix metalloproteinase inducer,EMMPRIN)在视网膜母细胞瘤(retinoblastoma,Rb)中的表达及其与临床病程的关系。方法用免疫组织化学SP法分别检测39例Rb中MMP-14及EMMPRIN的表达;用LEICA IM 50型全自动医学图像分析系统测量其平均灰度值,比较不同临床和病理阶段Rb中表达MMP-14和EMMPRIN的差异及二者阳性表达之间的关系。结果39例Rb中MMP-14阳性表达34例,占87.18%;EMMPRIN阳性表达33例,占84.62%;青光眼期Rb中MMP-14和EMMPRIN的表达明显高于眼内期(P<0.01及P<0.05),眼外期二者表达明显高于青光眼期(P<0.01及P<0.05);视神经浸润组Rb中MMP-14和EMMPRIN的表达明显高于无视神经浸润组(P<0.01及P<0.05);Rb中MMP-14和EMMPRIN的阳性表达水平呈密切相关(P<0.01)。结论MMP-14及EMMPRIN的表达水平可能与Rb的浸润转移有关,二者共同促进Rb的浸润转移。Objective To investigate the expressions of matrix metalloproteinase-14 (MMP-14) and extra-cellular matrix metalloproteinase inducer (EMMPRIN) in rednoblastoma(Rb) and the relationships between MMP-14 and EMMPRIN and tamor development. Methods Immunohistochemieal technique was used to determine the expressions of MMP-14 and EMMPRIN in 39 cases eft paraffin embedded Rb samples. Quantitative analysis of the expressions of MMP-14 and EMMPRIN were assessed by measuring the mean gray scale of Rb tissue with the LEICA IM 50 Color Pathologic Analysis System. Differences of the expreasions eft MMP-14 and EMMPRIN in each clinical and pathological stage were statistically analyzed, and the same step was also undertaken to study the relationship between Rb with MMP-14 positive expression and that with EMMPRIN positive expression, Results Positive expression rate of MMP-14 was 87.18% (34/39) and of EMMPRIN was 84.62%(33/39). The expressions of MMP-14 and EMMPRIN were significantly higher in tamors of the glaucomatous stage than those in the intraoeular stage ( P 〈0.01 and P 〈 0.05), and the same conclusion can be concluded between those in the extra-ocular stage and those in the glaucomatous stage ( P 〈0.01 and P 〈0.05), also the expressions of MMP-14 and EMMPRIN were significantly higher in tumors with optic nerve invasion than those without optic nerve invasion ( P 〈0.01 and P 〈0.05). There was a statistically significant positive correlation between the expression of MMP-14 and the expression of EMMPRIN ( P 〈 0. 01 ). Conclusions Positive expression levels of MMP-14 and EMMPRIN may correlate with tumor invasion and metastasis and they may facilitate the Rb' invasion and metastasis together
