准分子激光原位角膜磨镶术中眼球旋转及瞳孔中心移位的研究

摘　　要：目的探讨准分子激光原位角膜磨镶术(LASIK)检查位(坐位)与治疗位(卧位)的眼位、瞳孔变化规律及虹膜定位的重要性。方法57例近视眼患者共107只眼接受LASIK手术。术前利用VISX Wavescan波前像差仪获取患者检查位(坐位)的虹膜影像图,手术时掀开角膜瓣后用VISX STAR S4 IR准分子激光治疗机获得患者(卧位)虹膜影像图,比较两幅图的差异,计算出眼球的旋转度数和瞳孔中心移动的距离。结果106只眼发生旋转,107只眼瞳孔中心移位,86.5%的右眼发生外旋,63.6%的左眼发生内旋,眼球最小旋转0.0°,最大旋转9.2°。瞳孔中心大部向颞上和颞下象限移位。结论LASIK手术医师应重视术中普遍存在的眼球旋转运动和瞳孔中心移位,从而实现精确的个性化切削,避免偏中心切削及散光矫正偏差。OBJECTIVE To study the effect and the significance of the iris registration on correcting eye cycloduction and pupil centroid shift of eyes in Custom VueTM LASIK. METHODS One hundred and seven eyes of 57 consecutive myopic patients who had Custom VueTM LASIK were involved in the study. Wavefront measurement and iris reg- istration were performed in the upright position with Wavescan aberrometer, then more over iris registrations were performed in the supine position during the operation with VISX Star S4 IR. The differences between eye cycloduction and pupil centroid shift were analyzed. RESULTS （ 1 ） Cycloduction： One hundred and six eyes were extorsion when iris registration performed after lifting a flap（counterclockwise rotation in the right eye,86.5 % ,and clockwise in the left eye, 63.6 % ,the smallest cycloduction 0°, the biggest cycloduction 9.2°）. （2）Pupil centroid shift： One hundred and seven eyes were extorsion when iris registration performed after lifting a flap. The trends of the pupil centroid shift were towards temporal side either right eyes or left eyes. CONCLUSIONS Custom VueTM LASIK with iris registration may be a usefill tool for correcting eye cycloduction and pupil centroid shift. The iris registration should be done after liftiong a flap to get a better solution.
