抗幽门螺旋杆菌感染在特发性血小板减少性紫癜患者的应用研究

摘　　要：目的观察特发性血小板减少性紫癜（ITP）患者发生幽门螺旋杆菌（HP）感染的情况，探讨抗HP治疗ITP合并HP感染的应用效果。方法^14C-尿素呼气试验（^14C—UBT）检测，68例ITP患者中有40例感染HP，将40例感染者随机分为治疗组和对照组，每组各20例。治疗组给予根治性抗HP联合糖皮质激素等治疗，对照组未给予抗HP治疗，其它治疗相同。观察两组疗效及复发率，停药后随访1年。结果68例ITP患者HP的感染率约为58．82％，治疗组总有效率90％，对照组总有效率80％，随访1年治疗组复发2例（10％），对照组复发10例（50％）。结论进行根治性抗HP治疗，加强对ITP患者的护理，做好健康教育，可明显提高疗效，明显降低复发率，减少了住院时间和治疗费用。Objective To study the incidence of Helicobacter pylori （Hp） infection in patients with idiopathic thrombocytopenic purpura （ITP） and to evaluate the effects of Hp eradication in the treatment of ITP. Methods The incidence of Hp infection was detected by 14C urea breath test （ 14C - UBT） in patients with ITP, 40 of 68 patients were Hp positive. The patients who had positive result were randomly divided into two group： the treatment group （ n = 20） and the control group （ n = 20） . The treatment group were treated with Hp eradication combined with glucocorticoids, but the control group was treated with glucocorticoids only. The curative effect and recurrence were observed, and followed up after stopping the therapy for 6 months. Results The prevalecde of Hp infection in ITP patients in Shantou area was 58.82%, the total effective rate in the treatment group and the control group was 90% and 80% respectively. A fellow - up for 6 months showed that 2 cases recurrence in treatment group （ 10% ） and 10 cases in control group （50%） . Conclusion The study had shown the Hp eradication treatment could improve the therapy effect obviously, decline the relapse rate obviously and reduce hospitalization expense and length of stay in hospital.
