摘 要:目的:探讨磁共振扩散加权成像对结直肠癌肝转移的诊断价值。方法:27例结直肠癌患者行肝脏磁共振扩散加权成像、非增强磁共振和多层CT检查。阅片前告知阅片者患者为结直肠癌术后,但不提供临床病史和既往影像学资料。结果:多层CT、非增强磁共振和扩散加权成像对肝转移灶的敏感度分别为72%(45/63)、76%(48/63)和93%(58/63),扩散加权成像对肝转移灶具有更高的敏感度且与多层CT和非增强MRI相比,差异有显著性意义。扩散加权成像对肝转移患者敏感度最高(82%),而多层CT和非增强MRI分别为77%和66%。结论:磁共振扩散加权成像对结直肠癌肝转移灶的检出率高于多层CT和非增强MRI。Objective:To investigate the value of MR diffusion-weighted imaging (MR DWI) in the detection of liver metastases of colorectal carcinoma. Methods: 27 patients with liver metastases of colorectal carcinoma were examined with MR DWI,unenhanced MRI and multi-slice CT (MSCT). The images were read by radiologists who only knew that the patients had colorectal carcinoma,but didn't had any information of other clinical data and the previous imaging materials. Findings of unenhanced MRI, MR DWI and CT were compared. Results: Analysis based on lesion detection, the overall detection rate of liver metastases was 72% (45/63) for MSCT,76% (48/63) for unenhanced MRI,and 93% (58/63) for MR DWI. MR DWI was more sensitive than either MSCT or unenhanced MRI with significant statistical difference. Based on patient detection the detection rate was 82 % for DWI, 66 % for unenhanced MRI and 77 % for MSCT respectively. Conclusion: Diffusion-weighted MR imaging was superior to unenhanced MRI and MSCT in the detection of liver metastases of colorectal carcinoma.
