摘  要:目的:检测急性髓细胞白血病(AML)Bcl-2蛋白,并评价其与AML患者临床特征及多药耐药基因MDR1表达产物P170的关系。方法:免疫细胞化学ABC法检测白血病细胞中Bcl-2蛋白及P170表达。结果:AML中存在Bcl-2高水平表达,但复发的AML与初发AMLBcl-2表达无显著差异(P>0.05)。初发AML中,Bcl-2阳性组患者完全缓解(CR)率显著低于Bcl-2阴性组(P<0.01)。Bcl-2的阳性与阴性组中P170表达无显著差异(P>0.05)。Bcl-2和P170间无相关表达(rs=0.171,P>0.05),但Bcl-2/P170双阳性组化疗CR率显著低于双阴性组(P<0.01)或P170/Bcl-2单阳性组(P<0.05)。结论:初发AML中Bcl-2蛋白高水平表达与化疗耐药有关;Bcl-2与P170联合表达患者化疗反应更差。Objective:To detect the expression of Bcl-2 in acute myelocytic leukemia(AML),and evaluate the relationship between Bcl-2 and clinic characteristics of AML,and P170 product of multi-drug resistance genel (MDR1). Method:Immunocytochemical ABC assay was adopted to detect Bcl-2 and P170 protein in the cells. Results:The higher expression of Bcl-2 was found in AML(P〈0.05). But there was no significant difference of Bcl-2 expression between initial AML and relapsed AML(P〉0.05). The higher expression of Bcl-2 in initial AML was closely related to the lower complete remission (CR) rate (P〈0.05). The Bcl-2 expression was not correlated with the P170 expression in AML (r1= 0. 171 ,P〉0.05). But the CR rate in Bcl-2 P170 both positive group was significantly lower than Bcl-2 or P170 positive group (P〈0. 05) or both negative group (P〈0. 01 ). Conclusion:Bcl-2 and P170 can predict chemoresistance. The coexpression and P170 can predict chemoresistance more precisely.
