幽门螺杆菌感染与胃粘膜增生细胞核抗原及细胞凋亡关系的研究

摘　　要：为探讨胃粘膜幽门螺杆菌（Ｈｐ）感染与胃癌发生的关系，采用快速尿素酶法及免疫组织化学方法，检测１００例胃粘膜活检组织中Ｈｐ感染、增生细胞核抗原（ＰＣＮＡ）和细胞凋亡的状况。结果显示：慢性浅表性胃炎、慢性萎缩性胃炎、异型增生和腺癌，Ｈｐ感染率逐渐升高。定量分析发现，随病变加重，单位面积内ＰＣＮＡ免疫阳性细胞数逐渐升高，而细胞凋亡变化不明显。在同一类型病变中，Ｈｐ感染者细胞增生指标ＰＣＮＡ明显高于Ｈｐ阴性者，而凋亡指数却低于Ｈｐ阴性者。提示Ｈｐ感染在胃癌变过程中有重要的生物学意义。The　objective　of　this　study　was　to　investigate　the　correlation　between　Helicobacter　pylori(Hp)infection　and　gastric　carcinogenesis.Rapid　urease　and　immunohistochemical　methods　were　adopted　to　detect　Hpinfection　and　expression　of　proliferating　cell　nuclear　antigen(PCNA)and　apoptosis　in　gastric　mucosa　with　various　lesions.Results　showed　that　Hppositivity　increased　in　degree　with　chronic　superficial　gastritis,chronic　atrophic　gastritis,dysplasia　and　adenocarcinoma.The　number　of　PCNA　immunostaining　positive　cells/mm2　increased　with　progression　of　lesions,but　apoptosis　varied　slightly.However,in　a　similar　lesions,Hppositive　lesions　had　higher　PCNA　and　lower　apoptotic　index　than　Hpnegative.The　findings　indicated　that　Hpinfection　may　have　an　important　biological　significance　in　gastric　carcinogenesis.
