同期化放疗治疗中晚期食管癌的临床分析

摘　　要：目的探讨中晚期食管癌治疗的有效方案。方法中晚期食管癌70例随机分为2组：单纯放疗（单放组）35例，采用常规分割放疗，总剂量为60—70Gy／6—7W；同期化放疗（化放组）35例，应用CF／DF、单药紫杉醇化疗联合放疗。结果化放组总有效率（RR）为82．9％，单放组RR为60．0％（P〈0．05）；2组1、2、3年生存率分别为65．7％、40．0％、28．6％和54．3％、17．1％、8．6％，2,3年生存率2组有显著性差异（P〈0．05）。化放组的胃肠反应及骨髓抑制高于单放组（P〈0．05）。结论同期化放疗是治疗中晚期食管癌较为有效且安全的方案，毒副反应略增加，患者均可耐受。Objective To explore the effective regimen for patients with locally advanced esophageal cancer. Methods 70 patients with locally advanced esophageal cancer were randomly divided into the radiotherapy alone group （35 patients） and the concurrent chemoradiotherapy group （35 patients）. The radiotherapy was the same in the two groups, and was given in 200cGy per fraction,5 fractions per week with a dose of 6000 to 7 000 cGy in 6 to 7 weeks. CF,DF or Taxol was given to patients concurrently in the chemoradiotherapy group. Results The overall response rate （ RR ） in the concurrent chemoradiotherapy group was 82.9%, which was significantly higher than that in the radiotherapy alone group （60.0%, P 〈 0.05 ）. The 1,2 and 3 year overall survival rate was 65.7% ,40.0% ,28.6% and 54.3% ,17.1% ,8.6% ,respectively. The 2 and 3 year overall survival rates in the concurrent chemoradiotherapy group were signidicantly higher than that in the radiotherapy alone group （ P 〈 0.05 ）. The myelosuppression and gastrointestinal toxicities were significantly more common in the concurrent chemoradiotherapy group than that in the radiotherapy alone group （ P 〈 0.05 ）. Conclusion Radiotherapy combined with concurrent chemotherapy is an effective and safe modality in the treatment of patients with locally advanced esophageal cancer, with increased but tolerable toxicities.
