白内障术中后囊破裂Ⅰ期前房型人工晶状体植入术的观察

摘　　要：目的探讨白内障术中后囊破裂行Ⅰ期前房型人工晶状体植入的效果和安全性。方法27例(27眼)白内障术中发现后囊破裂、玻璃体脱入前房,经前段玻璃体切除后,植入PMMA弹性开放襻一体式前房型人工晶状体(AC-IOL)。结果术后1周及1个月视力≥0.3者分别为85.2%和88.9%,≥0.5者分别为70.4%和81.5%。术后并发症角膜水肿、葡萄膜炎和眼压升高均在1周内恢复正常,在襻的周围有局限性房角粘连。结论弹性开放襻一体式前房型人工晶状体植入术操作简单,视力恢复好,并发症较少,可作为后房型人工晶状体植入失败补救措施的首选方法。Objective To evaluate the effect and security of the AC-IOL （anterior chamber intraocular lens） implantation when the posterior capsule ruptured during the course of cataract operation. Methods Posterior capsule rupturing and vitreous body entering into anterior chamber were found during the course of cataract operations in 27 patients （27 eyes）, then their anterior vitreous bodies were cutted and the flexzible open-loop AC-IOLs made with PMMA were implanted. Results In 1 week and 1 month after the operation, there were 85.2% and 88.9% patients （ eyesight ≥ 0. 3） , and there were 70. 4% and 81.5% patients （eyesight ≥ 0. 5） respectively. Complications such as corneal edema, uveitis and increased introcular tension recovered in 1 week after operation. The local angle conglutination was found around the open-loop. Conclusions Flexzible open-loop AC-IOL implantation which has the excellence of the simple operation, better eyesight resumption and less complications, can be considered as the first re-medial choice when the posterior chamber intraocular lens implantation failured.
