尼美舒利导致多脏器功能不全综合征死亡

摘　　要：1名66岁男性患者因腰痛口服尼美舒利100mg,2次/d。服药7d后患者出现黄疸、皮肤瘙痒,之后逐渐出现呼吸困难、少尿、肝肾功能衰竭。甲、乙、丙型病毒性肝炎的血清试验均呈阴性。考虑为尼美舒利导致多脏器功能不全综合征。入院后,给予舒他西林、甲泼尼龙、谷胱甘肽及连续静脉-静脉血液滤过治疗,患者肝肾功能持续恶化,并出现消化道出血,入院第4d死亡。A 66-year-old man with lumbago was treated with nimesulide 100 mg twice daily. He developed jaundice and pruritus after 7 days of treatment, followed by dyspnea, oliguria, hepatorenal failure gradually. Serologic tests for viral hepatitis A, B, and C were negative. Nimesulide-induced muhiorgan dysfunction syndrome was suspected. After admission, he was treated with suhamicillin, methylprednisolone, glutathione, and continuous veno-venous hemofiltration. The patient＇s condition including hepatic and renal functions progressively worsened accompanied with gastrointestinal tract hemorrhage. He died on day 4 after admission.
