脑梗塞患者的颈动脉超声及颅内动脉血流动力学变化相关分析

摘　　要：目的:探讨应用彩色多普勒超声检测颈动脉及颅内动脉进行脑梗塞高危人群防治的临床实用价值。方法:112例经CT、MRI检查确诊的脑梗塞住院患者,另外选择58例体格检查正常者为对照组。采用彩色多普勒血流显像(CDFI)及经颅多普勒超声(TCD)对各组CDFI及TCD结果进行回顾性分析。结果:TCD检查结果:TCD对脑部血流供应不足的脑梗塞诊断有较明显优势。TCD对单支动脉狭窄所致脑梗塞也能早期进行诊断。CDFI检查结果:在TCD显示PI及RI增高,尤其是颅内动脉多处血流速度下降的病例当中,CDFI常表现为颈总动脉IMT增厚、粗糙且多伴有多发粥样斑块形成,两者有高度相关。结论:CDFI与TCD结果的综合分析可以对脑梗塞患者颅内、外动脉病变的形态学及血流动力学的改变进行客观的分析,对脑梗塞患者颅内、外动脉硬化病变的早期发现、早期治疗具有重要的临床意义。Objective： Discussion about the clinical value of Doppler Uhrasonography in the diagnosis of the carotid artery and Intracranial artery for the Prevention and Treatment of High Risk Population from those Cerebral Infarction patients. Methods： Disease Group is composed of 112 Cerebral Infarction inpatients who are diagnosed with CT and MRI. Control Group includes 58 people who are normal in physical examination. Retrospective Analyses of respective result are made using CDFI and TCD. Results： TCD Results：TCD has more distinctive advantage of the diagnosis on those Cerebral Infarction patients who are in cerebral blood flow shortage. TCD can also check out Cerebral Infarction resulted by the single artery Stenosis in early diagnosis. CDFI Results： When PI and RI are shown increased in TCD, especially the velocity of blood decreased in many parts of Intracranial artery, CDFI often represent with the incrassation and granulation of IMT in common carotid artery, mostly companioned with the formation of Atherosclerotic Plaque. These two methods have significant difference. Conclusion： the comprehensive analysis of CDFI and TCD resuits can diagnose objectively on the changes of morphologic and Hemodynamics of carotid artery and Intracranial artery for those Cerebral Infarction patients. It has significant clinical value to diagnose pathological changes of carotid artery and Intracranial artery atherosclerosis and give treatment in early phase for those high- risk populations.
