腹膜假性黏液瘤MR诊断(附17例报告)

摘　　要：目的：探讨腹膜假性粘液瘤的临床影像学表现，提高对本病的认识。方法：搜集经手术、病理证实的腹膜假性粘液瘤17例，回顾性分析MR表现。结果：17例中原发瘤为恶性6例，其中卵巢黏液囊腺癌2例，阑尾黏液囊腺癌3例，结肠黏液腺癌1例；良性11例，其中阑尾黏液囊腺瘤5例，阑尾黏液囊肿2例，卵巢黏液囊腺瘤3例，卵巢黏液囊肿1例。17例MR表现为①腹腔内均见直径1．9—30cm大的囊实性肿块，肝脾等实质脏器边缘“扇贝样”或“结节状”压迹5例；②脏器周围、网膜间隙、腹腔、盆腔见大量黏液性腹水；③15例腹膜、大小网膜弥漫性不规则增厚呈饼状，肠管受压移位，走行僵硬，厚度1．0—2．0cm不等；④MR信号在T1W上低信号，在T2W上呈高信号；④5例MR增强仅显示囊实性病变的囊壁、网膜、腹膜轻度强化，而囊内病灶无明显强化。结论：MR对显示腹膜假性粘液瘤具有一定的特点，是诊断腹膜假性粘液瘤较有价值的方法。Objective：To investigate the characteristics of imaging findings and clinical of peritoneal pseudomyxoma, in order to improve the diagnosis. Methods： 17 cases with pseudomyxoma peritonci had proceeded by operation and pathology. The findings were retro- spectively analysed of the MR, meanwhile discussed in reference to the pertinent literatures. Results：2 mutinous cystadenocarcinoma of ovary and 3 mucinous cystadenocarcinoma of appendiceal and 1 mucinous adenocarcinoma of colonic in malignancy of 6 cases; 3 appendiceal mucinous cystadenoma of ovary and 5 mucinous cystadenoma and 2 mucinous cyst of appendiceal and 1 mutinous cyst of ovary in benign of 11 cases. The MR findings are ① the compression mark of scallop shape and nodule account of border in hepatic and spleen, the multiple cystic mass intrapefitoneally; ② the large volumes of mutinous ascites in the parenchymal organ and abdominal cavity and pelvic cavity and omentum interspace; ③ the 1.0- 2.0 era of the increased thickness diffuse irregular and the shaped like a cake in the peritoneum and big and lesser omentum, the press and displacement and the stiffness intestine of 15 cases; ④ Hyperintense on T2WI and hypointeme on T1WI in 17 cases; ⑤slight enhaneemet of peritoneal and the cyst wall and omentum in 5 cases. Conclusion：There are some features in MR of pseudomyxoma peritonei, MR is an important method in diagnosing pseudomyxoma peritonei.
