摘 要:为证实正交设计在SRCA肿瘤药敏测定中的实际应用价值及远交系昆明种小鼠用于SRCA的可行性,本文采用正交设计观察研究了48例不同肿瘤在昆明种小鼠肾包膜下的生长情况及SRCA结果与临床疗效的相关性。结果表明,人体肿瘤在昆明种小鼠肾包膜下能够良好生长,实验总可评价率为89.78%。正交设计中R值的大小与临床疗效高度相关(P<0.005);当规定R≥100为敏感标准时,实验阳性符合率为84.62%(95%CI55-98%),阴性符合率为91.67%(95%CI73-99%),预测总准确率为89.19/(95%CI75-97%);含有SRCA敏感药物化疗方案的临床疗效显著高于不含SRCA敏感药物的化疗方案(P<0.001)。我们的研究结果提示在SRCA肿瘤药敏测定,采用正交设计只需常规随机分组,设计一半的动物用量,并保持与常规随机分组相同的准确性;人体肿瘤在昆明种小鼠肾包膜下能够良好生长,在SRCA肿瘤药敏测定中,完全可以采用来源广泛、价格低廉的昆明种小鼠代替价格昂贵的纯系小鼠。To investigate the practical value of orthogonal design as well as the posslbility of using hybrid Kunming mice to substitute inbrid mice, we observed the growth of the xenografts from 48 cases of various solid malignant tumor specimens under the renal capsule of Kunming mice and the corrlation between the SR-CA results and clinical therapeutic effect. In SRCA, we adopted orthogonal design instead of randomly design. The result demonstrated that human turnor xenograftscould grow well at the end of 4 days after implantation, the overall evaluability rate being 89. 78%, and that there was a significant correlation (PwtO. OO5) between the R value in orthogonally designed SRCA and clinical therapeutic effect, According to the criteria at our lab R≥100 was defined as positive for sensitlvity. Thus,the truely positive rate was 84. 62 % (95 % CI 55 ~ 98 % ), truely negative rate was 91. 67 % (95 % Cl 73 ~ 99% )and the overall predictive accuracy was 89. 19%(95 % Cl 75~ 97 % ). The regimens containing sensltive agents lndicated by the SR-CA yielded significantly better clinical therapeutic effect than that of reglmens without sensitive agents shown in the SRCA. Our data suggest that orthogonal design in SRCA can reduce the mice number by half and yet produced similar result as that by routine tompletely random design. Human tumor xenografts can be fostered under the renal capsule of Kunming mice just as well as in the expensive inbrid mice for SRCA.
