神经白塞氏病的临床和影像学分析

摘　　要：目的分析神经白塞氏病的临床和影像学改变,探讨该病的诊断和鉴别诊断。方法分析16例神经白塞氏病患者的临床和影像学资料;随访10例神经白塞氏病患者,对该病的演变规律进行总结。结果16例神经白塞氏病患者均有白塞氏病的基本临床表现,其中口腔溃疡16例,生殖器溃疡10例,眼部病变6例,皮肤病变8例,针刺反应阳性10例;以神经系统损害为首发症状者只有2例,其余14例从白塞氏病基本临床表现到出现神经系统症状的时间平均5.8年。16例中单纯中枢神经系统损害12例,周围神经损害2例,两者同时合并者2例。16例行头部MR检查均显示颅内散在点状或小片状T1加权像正常或低信号、T2加权像高信号影。结论神经白塞氏病在中枢神经系统的任何部位均可因小血管炎而受损,影像学改变有助于临床诊断,结合白塞氏病的基本临床表现及针刺反应综合分析可诊断神经白塞氏病。Objective To explore the methods of diagnosis and identification of Neuro-Behcet＇s disease through analyzing clinical manifestation and imaging alteration. Methods Clinical and imaging information of 16 cases of Neuro-Behcet disease were analyzed ,and 10 cases of patients were followed up. Then variational rules of Neuro-Behcet＇s disease were summarized. Results All the 16 cases of patients with Neuro-Beheet＇s disease had basic clinical manifestation of Behcet＇s disease. All the 16 cases were with dental ulcer, 10 cases with genital ulcer,6 cases with ocular affection ,8 cases with dermatogic disease, and 10 cases with positive acus response. There were only 2 cases of patients with neuro system damage as the leader symptom. There were about 5.8 years from basic clinal manifestation found to neural system damage in the other 14 cases of patients. In the 16 cases ,there were 12 cases with central nervous system symptoms and 2 cases with peripheral lesion. Nervous systemic and peripheral lesion were incorporated in the other 2 cases. Nomal or hyposignal of T1 weighting image and homosignal image of T2 weighting image with punctiforul or in fragmentis distributed diffusely in magnetic resonance of caruium. Conclusion Any location of central nervous system would be damaged because of polyangitis in patients with Neuro-Behcet＇s disease. The alteration in imageology would conduce to diagnose. Neuro-Behcet＇s disease would be diagnosed by aggreagete analysis of basic clinical manifestation bonding with acus response.
