摘　　要：目的探讨鞍区肿瘤患者手术前后血中抗利尿激素变化及其与水钠代谢紊乱的关系。方法在行鞍区肿瘤切除术的37例患者之中选取心肾功能正常的30例，对其手术前后抗利尿激素水平进行监测。根据研究对象的水钠代谢紊乱类型，将研究对象分为5组。结果术后无水钠代谢紊乱者（Normal）8例，手术前后抗利尿激素水平无统计学意义。尿崩症者（D1）15例，采血前注射垂体后叶素者（D1A）7例，术后抗利尿激素水平明显高于术前，有统计学意义；采血前未注射垂体后叶素者（D1B）8例，术后抗利尿激素水平明显低于术前，有统计学意义。脑性盐耗综合征者（CSWS）5例，手术前后抗利尿激素水平无统计学意义。抗利尿激素分泌不当综合征者（S1ADH）2例，术后抗利尿激素水平明显高于术前，有统计学意义。结论水钠代谢紊乱是鞍区肿瘤患者最常见的并发症之一，其与患者手术前后血中抗利尿激素的变化有密切联系。Objective To study the change of the serum antidiuretic hormone level before and after surgical excision of sella tumors and the relationship with water and sodium disorders. Method This research selected 30 patients from 37 patients of sella tumors between November 2006 and January 2007, and tested the serum antidiuretic hormone level before and after surgery. According to the types of water and sodium disorders, 30 patients were divided into 4 groups： the normal group（ Normal）, the group of diabetes insipidus（D1）, the group of cerebral salt wasting syndrome （CSWS）, the group of the syndrome of inappropriate secretion of antidiuretic hormone （ S1ADH ）. The group of diabetes insipidus （ D1 ） was divided into 2 groups： the group being treated with pituitrin （ D1A ） and the group not being treated with pituitrin （D1B）. Results There were 8 patients in normal group, not obviously different from the level before surgery;15 patients in the group of D1, 7 patients in the group of D1A, obviously higher than the level before surgery ; 8 patients in the group of DIB, obviously lower than the level before surgery ; 5 patients in the group of CSWS, not obviously different from the level before surgery ; 2 patients in the group of S1ADH, obviously higher than the level before surgery. Conclusions Water and sodium disorders is one of most common complications of the patients of sella tumors, hormone level before and after surgery.
