彩色多普勒超声对腮腺肿块的血液动力学研究

摘　　要：本文对41例腮腺肿块进行了彩色多普勒血流显像（CDFI）及脉冲多普勒（PD）的检测，其中腮腺癌11例，良性肿块3O例，均经手术和病理证实。结果表明：CDFI能迅速显示腮腺肿块的血流形态和分布。PD可区分动、静脉血流，在CDFI引导下进行检测可避免单纯PD检测的盲目性。动脉血流在10例腮腺癌和4例良性肿块中获得，而腮腺癌的血流阻力指数（RI）均大于0．55。彩色多普勒诊断腮腺肿块的总符合率可达975％（39／40），明显高于灰阶超声诊断。Forty-one cases with parotid masses were detected by color Doppler flow imaging and pulsed Doppler.11 malignant and 30 benign, were proved by operation and pathology. The state and distribution of color Doppler flow were readily displayed. The arterial and venous flows were differentiated by pulsed Doppler. Under the guidance of color Doppler flow image, the blindness detected by pulsed Doppler was avoided. The arterial signals were detected in 10 carcinoma cases and 4 benign cases. The flow resistant indexes of the cancers were over 0. 55. The diagnostic accuracy of parotid neoplasms with duplex color Doppler ultrasound was 97. 5%, higher than two-dimensional ultrasonography.
