肺癌组织中survivin的表达及其与细胞凋亡和增殖相关性的研究

摘　　要：目的探讨肺癌中survivin蛋白的表达与临床病理特征、肿瘤细胞凋亡和增殖的关系。方法采用SP法对83例肺癌组织和16例肺良性病变组织分别进行survivin及Ki67的免疫组化染色,分析survivin与肿瘤临床分期和肿瘤细胞分级间的关系,同时用原位末端标记法(TUNEL)标记肺癌组织中的凋亡细胞,并对survivin与凋亡指数(apoptosie index,Al)和增殖指数(proliferative index,PI)的相关性进行了研究。结果Survivin蛋白在16例肺良性病变组织仅能有1例表达(6.25%)。83例肺癌组织中survivin蛋白阳性表达率为62.7%(52/83),两组差异显著(P<0.05);survivin在肺癌中的表达与肿瘤的组织学分型、病理分级和淋巴结转移均无明显相关性(P>0.05),但与临床分期(TMN)有相关性(P<0.05)。Survivin阳性组平均AI值显著低于阴性组(P<0.01),survivin凋亡指数表达与AI值呈负相关(r=-0.6008,P<0.01);Survivin阳性组平均PI值显著高于阴性组(P<0.05),survivin表达与PI值呈正相关(r=0.4885,P<0.01)。结论Survivin在肺癌组织中表达明显上调,可抑制肿瘤细胞凋亡、促进细胞增殖,在肺癌的发生、发展中起到重要作用。Objective To investigate the expression of survivin in lung cancer, and to evaluate the relationships between survivin protein and elinieopathologie characteristics, and cell apoptosis or proliferation. Methods Expressions of survivin and Ki67 were immunostained in tumor tissues of 83 patients, and TNUEL method was used to detect the cell apoptosis. The correlations between expression of survivin and eliniealpathologieal characteristics,cell apoptcsis and proliferation were assessed.Results The positive expression rotes of survivin in lung cancer and lung benign lesion tissues were 62.7% and 6.25% respectively, there was a significant difference between them（ P 〈 0.05）. Survivln expression in hmg cancer was not related to the histologictype, pathological grade and lymphnode metastasis（ P 〉 0.05）, but correlated with TNM stage （ P 〈 0.05）. The average apoptosis index（AI） of survivin-pesitive cases was significantly higher than that of survivln-negative ones（3.93 ± 1.46 vs 2.36 ± 1,21 ） （ P 〈 0,01 ）. There were negative correlations between suvivin expression and AI（ r = - 0. 6008, P 〈 0.01 ）. The average prollfemtion index（AI） of survivin-positive cases was sig- nificantly lower than that of survivin-negative ones（32.47 ± 22.31 vs 47.31 ± 28.06） （ P 〈 0.01 ）. There were positive correlations between suvivin expression and PI（ r = 0. 4885, P 〈 0.01 ）. Conclusion Overexpression of survivin in lang cancer cells may inhibit cell apoptosis and promote cell proliferation, and it was suggested survivin may be involved in oncogenesls.
