摘　　要：目的:评价小剂量5-氟尿嘧啶化疗同步放射治疗老年食管癌的疗效和副作用。方法:89例不能手术或不愿手术的食管癌老年住院患者,随机分为联合组(44例)和对照组(45例)。两组放疗均采用常规分割,前后对穿野加斜野,总剂量为DT60Gy,联合组放疗同期接受化疗,5-Fu 300mg/m2静脉注射,1次/d,每周5次,持续4-5周;对照组行单纯放疗。结果:治疗结束后,联合组与对照组总有效率(CR+PR)分别为81.8%和61.4%(P<0.05);1年生存率分别为77.3%和53.3%(P<0.05);2年生存率分别为70.5%和44.4%(P<0.05)。不良反应主要为放射性食管炎和白细胞下降,余反应轻微,组间无统计学差异(P<0.05),不影响治疗。结论:对于老年食管癌患者,低剂量5-氟尿嘧啶化疗联合放疗治疗可显著提高局部控制率和近期疗效,副作用轻微,值得推广。Objective：To explore the therapeutic effectiveness and evaluate toxicity of low - dose 5 - Fu combined with radiotherapy for advanced esophageal cancer in elder patients. Methods： Total of 89 elder patients with advanced esophageal cancer were enrolled into the research, they were inoperable or refused operation. The patients were randomized into two groups ：45 cases in the control group received radiotherapy alone while other 44 cases in the combined group additional plus 5 - Fu 300mg/m^2 ,5 days a week and continued for 5 -6 weeks . Radiotherapy was given with conventional fraction,the total tumor dose were 60Gy -70Gy. Results： The overall response （CR ＋ PR） rates for the combined group and the control group were 81.8% and 61.4% （ P 〈 0.05 ）, one - year survival rates were 77.3 % and 53.3 % （ P 〈 0.05 ） ; two - year survival rates were 70.5 % and 44.4% （ P 〈 0.05 ）, The adverse reactions were mainly radiation esophagitis and leukopenia;other side effect was mild and no statistic difference was found between two groups. Conclusion： To elder advanced esophageal cancer patients, low - dose 5 - Fu combined with radiotherapy could significantly improve local control rate and survival ;it had slight side effects and was economic. Therefore ,the combined regimen is worthy for widely use in elder patient with advanced esophageal cancer.
