微小钙化在乳腺疾病X线诊断中的意义

摘　　要：目的:分析乳腺良恶性微小钙化的X线表现形式,提高鉴别诊断能力。方法:回顾性分析了156例次乳腺微小钙化的X线表现,并与手术病理结果进行对照。结果:(1)乳腺癌103例次,(导管内癌55例次、导管癌早期浸润12例次、浸润性癌36例次);乳腺良性病变53例次。(2)多种钙化形态同时出现的87例次(乳腺癌79例次,良性病变8例次);钙化形态较单一的35例次(乳腺癌24例次,良性病变11例次)。另34例次良性病变表现为点状钙化中夹杂极少量其他形态钙化。(3)钙化呈单簇分布的35例次(乳腺癌14例次,良性病变21例次);钙化沿导管走行方向分布的54例次(乳腺癌42例次,良性病变12例次);钙化呈区域性分布50例次(乳腺癌38例次,良性病变12例次)。钙化弥漫分布整个乳腺的17例次(乳腺癌9例次,良性病变8例次)。结论:多种钙化形态同时出现,密集沿导管分布于乳腺内是钙化型乳腺癌的典型X线征象。其中钙化的形态、分布方式及密集度最具有诊断价值。Objective： To analyse mammographic feature of breast benign or malignant microcalcifications and improve identifying diagnosis of benign or malignant lesions. Methods： A retrospectively analysis was undertaken to compare 156 breast microcalcifications feature with pathologic finding, Results：（1） 103 cases of breast carcinoma （included ductal cancer in situ 55/103, ductal carcinoma with early infiltration 12/103, infiltrative carcinoma 36/103） and 53 cases of breast benign lesions were diagnosed. （2） Multimicrocalcification shapes appeared in the same time （n=87） （including 79 cases of breast carcinoma,8 cases of breast benign lesions）; single calcification shape appeared in 35 cases （including 24 cases of breast carcinoma,11 cases of breast benign lesions）; few other shape calcification with in punctate calcification appeared in another 34 cases.（3） Microcalcification was distributed like cluster （n=35）（included 14 cases breast carcinoma and 21 cases breast benign lesions）.Microcalcification was distributed along the duct（n=54） （included 42 cases breast carcinoma and 12 cases breast benign lesions）. Microcalcification was distributed in a region （n=50）（included 38 cases breast carcinoma and 12 cases breast benign lesions）.Microcalcifications were diffusely distributed in the whole breast （n=17）（included 9 cases breast carcinoma and 8 cases breast benign lesions）.Conclusion： The manner of Multi-shape of the microcalcification appear in the same time, along ductal denseness distribution is the typical X-ray sign of the breast cancer. The shape of the microcalcification and the dense degree and the manner of the distribution have the important value of the diagnosis.
