放疗联合5-Fu和顺铂治疗晚期鼻咽癌临床观察

摘　　要：目的观察放疗联合氟尿嘧啶(5-Fu)+顺铂(DDP)治疗晚期鼻咽癌的临床疗效。方法1998年9月至2002年10月对36例Ⅲ、Ⅳa期鼻咽癌患者治疗应用氟尿嘧啶+顺铂联合放疗,并与同期行单纯放疗的28例患者进行对比分析。结果化疗+放疗组和单纯放疗组鼻咽肿瘤完全消退率分别为91.7%和85.7%(P<0.05);颈部转移淋巴结完全消退率分别为63.9%和50%;两组3年生存率分别为63.9%和46.4%(P<0.05)。结论氟尿嘧啶+顺铂化疗和同期放疗等综合治疗能提高晚期鼻咽癌患者的生存率、局部控制率,减少远处转移。Objective To evaluate the clinical efficacy of radiation therapy combined with chemotherapy with cisplatin （DDP） plus 5-fluorouracil （5-Fu） in treatment of advanced nasopharyngeal carcinoma. Methods Thirty-six cases of stage Ⅲ, Ⅳa nasopharyngeal carcinoma received radiation therapy combined with chemotherapy with cisplatin plus 5-Fu, compared with 28 cases of radiation therapy alone at the same time. Results The complete remission rates（CR） in chemotherapy plus radiation therapy group and radiation therapy alone group were 91.7% and 85. 7% respectively（P 〈0. 05） ,the CR rates of neck lymph node metastasis were 63.9% and 50. 0% and 3-year survival rates were 63. 9% and 46.4% respectively （ P 〈 0. 05）. Conclusions Radiation therapy combined with chemotherapy cisplatin plus 5-Fu can improve the survival rate and local control rate of the patients with advanced nasopharyngeal carcinoma, and decrease metastasis.
