沙培林在肺癌系统淋巴结清扫手术中的应用

摘　　要：目的研究沙培林在肺癌系统淋巴结清扫手术中的应用效果。方法将176例行肺癌系统淋巴结清扫的患者随机分为两组,试验组在关胸前取沙培林10KE吸附于明胶海绵上,贴于淋巴结清扫后所余残腔内及肺切除残端。统计患者术后3d体温、术后3d胸腔引流量、住院天数、术后5年生存率及术后局部复发率,研究沙培林在肺癌系统淋巴结清扫手术中的应用效果。结果试验组患者术后3d高热发生率高于对照组(P<0.05),试验组术后3d胸腔引流量及住院天数均少于对照组(P<0.05);两组患者术后5年生存率比较差异无显著性(P>0.05),试验组局部复发率明显低于对照组(P<0.01)。结论肺癌系统淋巴结清扫手术中应用沙培林可引起术后早期高热,但可以减少引流量及住院天数,并可降低局部复发率,在肺癌系统淋巴结清扫手术中有一定的使用价值。Objective To investigate the efficacy of sapylin in systemic lymphadenectomy of lung cancer. Methods 176 cases undergoing systemic lymphadenectomy of lung cancer were divided into two groups at random. In the test group 10KE sapylin absorbed by gelatin sponge was put into the cavity after lymphadenectomy and the stump after resection of lung. Three days after operation, body temperature, drain- age volume in 3 days after operation, days of hospital stay, five-year survival rate and local relapse rate were determined to investigate the efficacy of sapylin. Results The rate of high fever during 3 postoperative days in test group was higher than that in control group（ P 〈 0. 05 ）. Drainage volume during 3 postoperative days and days of hospital stay in test group were less than those in control group（ P 〈 0. 05 ）. There was no difference in the five-year survival rate between the two groups （ P 〉 0. 05 ）. The local relapse rate in test group was lower than that in control group（ P 〈 0. 01 ）. Conclusions The use of sapylin in systemic lymphadenectomy of lung cancer could cause high fever in early postoperative period ,but could reduce the drainage volume, days of hospital stay and local relapse rate, so it has value for systemic lymphadenectomy in operation of lung Cancer.
