玻璃体切除联合Ⅰ期人工晶体植入术治疗外伤性晶状体后脱位的临床分析

摘　　要：目的探讨玻璃体切除手术治疗外伤性晶状体后脱位的临床疗效。方法对32例因外伤致晶状体脱位于玻璃体腔的患者,采用标准睫状体扁平部三切口闭合式玻璃体切除术直接切除晶状体,或注入全氟化碳液体经角膜缘取出晶状体。再植入前房型人工晶体、虹膜固定型人工晶体或后房型睫状沟缝线固定的人工晶体。结果32眼脱位于玻璃体腔之晶状体被成功取出,28例(87.5%)患者术中Ⅰ期植入人工晶体,术后视力均有不同程度的提高。结论脱位于玻璃体腔的晶状体,采用玻璃体切除术尽早取出,同时植入人工晶体,术后并发症少,视功能恢复较为满意。Objective To evaluate the efficacy of the treatment in vitrectomy for posteriorly dislocated lens after trauma.Methods Standard pars plana vitrectomy were used in 32 eyes whose lens dislocated into the vitreous cavity.Lens were removed through the limbus of cornea after infused perfluorocarbon fluid infusion,and some lens were fragmented in the vitreous cavity.Anterior intraocular lens or ciliary sulcus fixation lens were implanted at the same time.Results The dislocated lens were successfully removed in all these 32 eyes.Introcular lens were implanted in 28 eyes（87.5%）,and the visual acuity increased at different level in all patients.Conclusion The lens that dislocate into the vitreous cavity should be given vitrectomy as early as possible.Intraocular lens implantation at the same time is recommended.This treatment could improve visual function and decrease complications.
