摘  要:目的:探讨急性髓细胞白血病(AML)免疫分型和FAB分型的关系。方法:对93例初治AML进行细胞形态学检查和免疫分型检查,免疫分型采用流式细胞术(FCM),并对2种检查结果进行分析。结果:93例AML中M1亚型1例,M2亚型68例,M3亚型13例,M4亚型3例,M5亚型4例,未分类4例;单表型74例,伴淋系抗原表达12例,双表型5例,未表型2例;细胞形态学确诊AML72例,诊断准确率为77.4%;免疫分型确诊AML91例,FAB分型结合免疫分型使诊断的准确率达到97.8%。结论:免疫分型在鉴别AML各亚型和杂合性白血病(HAL)方面具有重要作用,细胞形态学结合免疫分型可提高诊断准确率,弥补FAB分型的不足之处。Objective: To study the relationship of immunophenotype and FAB phenotype in AML. Methods: Cell morphology checking and immunophenotyping were performed at the same time for the 93 initial treatment patients with AML, flow cytometry had used for immunophenotype, on the end the results had been analyzed to understand the relationship between the two phenotypes. Results: Among the 93 cases with AML, there were M1(1), M2(68), M3(13), M4(3), M5(4) and UAL(4) in FAB phenofype, and AML(74) ,Ly+ AML (12), HAL (5) and UAL(2) in immunophenotype. The diagnostic accuracy of FAB phenotype was 77.4% (72/93); immunophenotype confirm 91 patientls diagnosis, FAB phenotype plus immunophenotype can improve AML's diagnostic ratio to 97.8%(91/93). Conclusions: Immunophenotype plays an important role to distinguish AMLIs subtype and HAL. It could improve the diagnostic accuacy and retrieve FAB phenotype's insufficiency.
