小切口白内障囊外摘出术中角膜后弹力层脱离的原因及处理

摘　　要：目的:分析白内障手术致角膜后弹力层脱离的原因,提出预防及处理方法。方法:对375例白内障施行小切口非乳化白内障囊外摘出人工晶状体植入术,术中27例(27眼)各种原因造成角膜后弹力层脱离者采用前房内注入平衡液、粘弹剂或空气泡对症处理。结果:分析白内障手术中由于切口的方式、宽度、位置及手术器械的机械性损伤等原因,处理了造成角膜后弹力层不同程度的脱离。结论:术中及早发现和及时行角膜后弹力层脱离复位,对预防术后的并发症及视力的恢复均起到重要作用。Objective：To explore the causes of corneal descemet membrane detachment in surgery for management of the events.Methods： 375 patients with cataract undergone small incision extracapsular cataract extraction and IOL implantation were examined for the management of descemet membrane detached in 27 patients,if there was the accidents,the anterior chamber with BES or medical sodium hyaluronate gel or air bubble would be introduced.Results： Incision type,length,location and mechanical lesion were responsible for the factors of corneal descemet membrane detachment in cataract operation Conclusion： Early diagnosis and treatment are important for preventing postoperative complication.
