摘 要:目的观察肝素对窒息性心跳骤停大鼠心肺复苏效果及脑组织超氧化物歧化酶(SOD)、丙二醛(MDA)的影响。方法50只雄性SD大鼠,随机分为三组:假手术组(C组)10只,肾上腺素组(A组)20只,肾上腺素+肝素组(H组)20只。麻醉后行气管插管和动静脉置管,A组和H组建立窒息性心跳骤停模型,窒息8min后开始心肺复苏,A组给予肾上腺素0.01mg/kg+与H组肝素等量的生理盐水,H组给予肾上腺素0.01mg/kg+肝素0.5mg/kg,C组不窒息、不复苏。记录窒息前、ROSC后1、5、10、30、60min MAP和HR,复苏后24h取脑组织测SOD活性、MDA含量。结果各组基础指标差异无统计学意义(P>0.05);与A组相比,H组复苏时间明显缩短(P<0.01),复苏后各时点MAP升高,复苏成功率提高(P<0.05)。复苏后24h,与C组相比,A组和H组SOD均降低,MDA均增多(P<0.01);H组SOD高于A组(P<0.05),MDA低于A组(P<0.01)。结论两组大鼠复苏后脑组织中SOD活性均减低,MDA含量增加,提示应用肝素可减轻自由基代谢失衡,减轻复苏后脑组织的损伤。Objective To observe the effects of heparin on the efficacy of cardiopulmonary resuscitation(CPR) in rats following asphyxia - induced cardiac arrest and the changes of superoxide dismutase (SOD) activity and malondialdehyde(MDA) contents in cerebral tissue of rats. Methods 50 male Sprague- Dawley rats were ran- domly divided into 3 groups: sham operation group( group C, n = 10), epinephrine group(group A, n = 20), Heparin group (group H, n = 20). The rat model of asphyxial cardiac arrest was established in group A and group H. CPR was started 8 minutes after asphyxiation. Epinephrine 0.01mg/kg combined with 0.9% NaCl or heparin 0.5mg/kg was administrated in group A and H, respectively. Group C was not established the model of CPR. The mean arterial pressuse (MBP) and heart mte(HR)was recorded before asphyxia, 1,5,10,30,60min after restoration of spontaneous circulation (ROSC). SOD and MDA was determined in cerebral tissue 24h after ROSC. Resuits There were not significant differences in baseline values among three groups ( P 〉 0.05). Compared with group A,time of ROSC was significantly decreased( P 〈 0.01 ), MAP and the resuscitation rates was increased and after ROSC( P 〈 0.05) .Compared with C 24h after ROSC, SOD was significantly decreased and MDA was significantly increased in both group A and H( P 〈 0.01).SOD and MDA in group H were higher( P 〈0.05) or lower( P 〈 0.01 ) than that in group A( P 〈 0.05). Conclusion Heparin has beneficial effects in enhancing the achievement of CPR, reducing the injury of brain and improving the cerebral function.
