云南红药胶囊治疗应激性胃溃疡出血的疗效观察

摘　　要：目的观察云南红药胶囊对应激性胃溃疡出血的疗效。方法选择我院2004年8月—2007年10月各种原因引起的应激性消化道出血病人148例,随机分为观察组58例和对照组90例,两组病人均常规输血、补液、抑制胃酸分泌、对症治疗。对照组在此基础上使用4℃的0.9%氯化钠溶液30ml加去甲肾上腺素8U经胃管注入,夹管30min,若未止血,每隔8h重复上述方法,持续48～72h。观察组在常规治疗的基础上给予云南红药胶囊口服治疗,2粒/次,3次/d。留置胃管者云南红药0.5g加入0.9%氯化钠溶液30ml中混匀,注入胃管,夹管30min,3次/d。观察两组病人的疗效及不良反应。结果两组病人临床疗效间差异有统计学意义（P〈0.05）。观察组用药后无发热、皮疹、口唇及肢体麻木、头晕、胸闷、心悸等不良反应及再出血,对照组第一次出血停止后1～11d有5例再出血。两组病人病死率间差异无统计学意义（P〉0.05）。结论云南红药直接作用于出血部位,能使血小板黏附、聚集,加速凝血酶生成,从而缩短了凝血过程,可保护胃黏膜,促进溃疡愈合。Objective To observe the curative effect of Yunnan Hongyao capsule on hemorrhage of upper digestive tract with stress ulceration.Methods Totally 148 patients with digestive tract hemorrhage caused by various irritable stimuli from August 2004 to October 2006 were randomly divided as the trial group （n=58） and the control group （n=90） group.The patients in both groups received conventional treatment of blood transfusion,fluid infusion,inhibition of gastric acid secretion and systematic therapy.Besides,the patients in the control group were treated with 30 ml icy sodium chloride solution plus 8 U noradrenalin by gastric canal,then the canal was pressed from both sides for 30 minutes,the procedure was repeated every 8 hours for 48 to 72 hours,if the hemorrhage was not stopped; while the patients in the control group received 2 capsules of Yunnan Hongyao,three times daily or 0.5 g Yunnan Hongyao in 30 ml of 0.9 % sodium chloride solution through gastric canal and then the canal was pressed for 30 minutes,three times daily.The effects and side effect in both groups were observed.Results There was no significant difference in clinical effect between the two groups （P〈0.05）.After treatment in trial group no side effects of fever,rash,numbness of lips and limbs,dizziness,chest distress and palpitation,as well as re-hemorrhage were found.There were 5 cased in the control group who got re-hemorrhage 1 to 11 day after the cessation of hemorrhage for the first time.There was no statistically difference in mortality between the two groups （P〉0.05）.Conclusion Yunnan Hongyao capsule can act on the bleeding site directly to reduce the process of blood coagulation,protect the gastric mucosa and promote ulcer healing.
