低位直肠癌行局部切除术与经腹会阴联合切除术的疗效比较

摘　　要：目的对比观察低位早期直肠癌行局部切除术(TAR)与经腹会阴联合切除术(APR)的临床效果。方法42例低位早期直肠癌患者随机分为两组:TAR组切除肿瘤及肿瘤边缘正常组织1cm,深度达直肠肌层部分切除;APR组按标准Miles手术操作。比较两组手术方式、并发症及随访结果。结果两组患者的手术时间、出血量、切除标本长度及并发症发生率间差别均无统计学意义(P>0.05);TAR组局部复发率10.0%,远处转移率5.0%,2年生存率95.0%;APR组局部复发率4.5%,远处转移率9.0%,2年生存率95.5%,两组间差别均无统计学意义(P>0.05)。结论低位早期直肠癌根治性局部切除手术可以取得与腹会阴联合切除术相当的疗效。Objective To observe the clinical value of abdominal peritoneum resection （APR） and low - set rectal cancer transanal resection （TAR）, Methods 42 patients were divided randomly into two groups： TAR group and APR group. Turnouts were resected, so were normal tissues around tumours by 1 cm and part of rectum muscle in TAR group; surgery in accordance with Miles standards was performed on APR group, We compared the surgical procedures, complications and follow - up results of the two groups, Results There was not significant difference in the indexes of surgical produre and incidence rate of complications between the two groups （ P 〉0. 05）. In both TAR group and APR group, local recurrence rates were 10 0% and 4 5%, rates of distant metastasis 5 0% and 9 0%, 2 - year survival rates 95 0% and 95 5%, respectively; Conclusion Early partial low - set rectal cancer radical surgery can get the same effect as abdominal perineal resection,
