摘　　要：目的:探讨急性胰腺炎肝功能损害程度及其临床意义。方法:通过对141例急性胰腺炎患者临床资料回顾性分析,按病情轻重分为重症胰腺炎和轻症胰腺炎,按病因分为胆源性及非胆源性。分析其肝功能指标血清丙氨酸转氨酶、门冬氨酸转氨酶、总胆红素、碱性磷酸酶、谷氨酰转移酶、白蛋白及其对疾病转归的影响。结果:急性重症胰腺炎患者肝功能损害程度明显比急性轻症胰腺炎患者重,胆源性胰腺炎患者肝功能变化亦明显重于非胆源性胰腺炎患者,且急性重症胰腺炎以胆源性者居多,住院时间较长。结论:急性胰腺炎早期即可能发生肝功能损害且影响预后。Objective To investigate the damage of the liver function in patients with acute pancreatitis and its clinical significance. Methods A total of 141 patients with acute pancreatitis were retrospectively analyzed. These patients were divided into two groups, severe acute pancreatitis and mild acute pancreatitis according to the degree of disease, and as well as they were divided into biliary pancreatitis group and non-biliary pancreatitis group according to the cause of disease. The changes of the markers of liver function were analyzed including alanine aminotransferase, aspartate aminotransferase, total bilirubin, alkaline phosphatase, r-glutamyl transferase and albumin. The effect of the changes of liver function on the prognosis of acute pancreatitis was analyzed too. Results The damage of liver function in patients with sever acute pancreatitis were more severe than that in the patients with mild acute pancreatitis. The damage of liver function in patients with biliary pancreatitis was more serious than that in patients with non-biliary pancreatitis significantly. I3iliary pancreatitis accounted the most in the patients with severe acute pancreatits. The time they were in hospital was longer. Conclusion The damage of liver function could happen in the early stage in patient with acute pancreatitis and the damage can affect the prognosis of acute pancreatitis.
