摘  要:目的探讨端粒酶hTRT mRNA在口腔鳞癌及癌前病变组织中的表达相关性。方法采用原位杂交法检测病变组织中的hTRT mRNA的表达水平,标本取自哈尔滨医科大学第二附属医院口腔颌面外科病理室1980年至2000年的病理标本,其中口腔鳞癌46例、非典型增生18例、单纯性增生5例、肿瘤周围邻近组织5例、正常口腔黏膜2例,共76例口腔病理标本。结果80.5%鳞癌(37/46)、55.6%非典型增生(10/18)、20%单纯性增生(1/5)、20%肿瘤周围邻近组织(1/5)、0%正常口腔黏膜(0/2),端粒酶hTRTmRNA显示阳性表达。其中口腔鳞癌与非典型增生之间端粒酶hTRTmRNA表达差异有统计学意义(P<0.05),重度非典型增生与轻度及中度非典型增生之间端粒酶hTRT mRNA表达差异有统计学意义(P<0.05),口腔鳞癌各临床分期之间差异无统计学意义(P>0.5)。结论端粒酶hTRTmRNA在口腔鳞癌及癌前病变中均有表达,端粒酶活性的激活可能发生在口腔癌前病变晚期,其活性的激活是口腔癌前病变恶变及恶性肿瘤形成的重要因素。Objective To investigate the implications of telomerase hTRT mRNA expressing level in the tissues of oral squamous cell carcinoma and oral precancerous lesion. Methods Seventy-six cases were detected by in situ. hybridization technique for hTRT mRNA including 46 cases of oral squamous cell carcinoma, 18 cases of dysplasia lesions (precancerous lesion), 5 cases of hyperplasia lesions, 5 cases of normal tissue around tumor,2 cases of normal mucosa. Results The positive expressing rates of hTRT mRNA were 80.5% (37/46) ,55.6% (10/18) ,20% (1/5), 20% (1/5) and 0% (0/2) for the groups of oral squamous cell carcinoma, dysplasia lesions, simple hyperplasia, paracancerous tissue and healthy oral mucosa respectively, with very significant difference between the group of oral squamous cell carcinoma and precancerous lesions. Furthermore, there was a significant difference in the expressing level of hTRT mRNA among the cases with severe degree of dysplasia and those with moderate degree of dysplasia ( P 〈 0. 05 ). Conclusions The expression level of hTRT mRNA has been significantly elevated in both oral precancerous lesions and oral squamous cell carcinoma. Activation of telomerase may occur at a late stage of precancerous lesion. Then, it will promote developing of oral precancerous lesion into malignant change, even leading to a malignancy forming.
