弧形切割吻合器在直肠癌低位前切除双吻合术中的应用

摘　　要：目的评价弧形切割闭合器在低位结直肠癌切除双吻合术中的应用。方法回顾性分析2004年10月至2006年8月对138例直肠癌低位前切除双吻合术,其中76例用直线闭合器行双吻合术,62例应用弧形切割吻合器行双吻合术。结果全组吻合口漏发生率为4.3%(10/138),无手术死亡,直线闭合器行双吻合术的吻合口漏发生率为11.8%(9/76),吻合口距肛门平均距离为7.0cm,应用弧形切割吻合器行双吻合术吻合口漏发生率为1.6%(1/62),吻合口距肛门平均距离为5.5cm。结论弧形切割吻合器的应用能扩大直肠癌保肛手术的应用范围,减少并发症,提高患者术后生活质量。Objective To evaluate the role of curved cutter stapler in double stapling technique of low rectal carcinoma. Methods Retrospective study was conducted to analyze the experience of double stapling technique in colorectal anatomosis for 138 cases with rectal carcinoma from October 2004 to August 2006. Results Complications included anastomotic leakage in 10 case（4.3 % ）, the death rate of the operation is zero, anastomotic leakage of curved cutter stapler is 1.6%, significantly lower than Strightline closer, and the average distance between anastomotic stoma and anus is 5.5 Centimeter. Conclusion Curved cutter stapler can amplify the indication of Sphincter - preserving operation for low rectal carcinoma, reduce complications, and increase the quality of life.
