摘 要:夜间酸突破(NAB)现象在抑酸治疗上消化道酸相关性疾病中有较高的发生率,其发生机制尚未完全明确,主要是夜间质子泵更新激活。目前的质子泵抑制剂(PPI)血浆半衰期短,仅对活化的质子泵产生抑制,夜间新生的质子泵,在PPI血药浓度下降后激活,逃逸PPI的作用;影响其产生的因素多,对其治疗目前仅有睡前加服H2受体拮抗剂能短期有效抑制NAB的发生,新型的质子泵抑制剂问世将会改变这一局面。There is a high incidence rate of nocturnal acid breakthrough ( NAB ) in the patients with acid related diseases treated with acid suppressant. The mechanism is not definite now, some reports suggested that its possible mechanism should be associated with that the proton pumps refresh and are activate in light. The plasma half-lifes of the available proton pump inhibitors are short. And these PPIs have effects on the activated proton pump only. The refreshed proton pumps are activated after the blood drug levels decrease, and escape the effects of PPIs. The influencing factors are numerous. At present, the treatment of ad- ministration with H2RA before retiring can effectively inhibit the occurrence of NAB. The appearance of neotype PPIs will change the situation.
