23例外周血干细胞采集过程中不良反应及其预防处理措施

摘　　要：目的探讨外周血干细胞采集过程中出现的不良反应及其预防处理措施。方法对23例患者或供者，使用MCS^＋血细胞分离机采集外周血干细胞，对采集过程中出现的不良反应进行观察、分析和处理。结果每例采集1～2次，共44次，出现不良反应24次（54．55％），其中枸橼酸盐毒性反应15次，血流通路障碍6次，低血容量反应1次，回输过快反应1次，微量渗血1次。采取相应处理措施后，不良反应消失，23例均能采集到足量的单个核细胞和CD34^＋细胞。结论运用MCS^＋血细胞分离机采集PBSC总体上是安全的，但不良反应时有发生，其中以枸橼酸盐毒性反应最多。Objective To investigate the adverse effects in the collection of peripheral blood stem cells（PBSC） and their prevention and cure methods. Methods PBSC were collected in twenty three patients or donors by blood cell separator MCS^＋. The adverse effects were observed,analyzed and cured in the collection of PBSC. Results The adverse effects occurred in 23 of 44 collec tions（54. 55%）, 15 of citrate poisoning,6 of blood flow problems, 1 of hypovolaemia, 1 of fast reinfusion problem,and 1 of trace errhysis, and disappeared after treatment. Sufficient quantum mononuclearcells and CD34^＋ cells were collected in twenty-three patients or donors. Conclusion Collection of PBSC by MCS^＋ apheresis machines is generally regarded as a safe procedure,however, adverse effects in PBSC harvesting are a common thing. The most of adverse effects in the collection of PBSC was citrate poisoning. The pre collection preparation and the prevention and cure of adverse effects should be required for ensuring successful collection.
