胃镜下微波烧灼联合局部化疗治疗晚期食管癌疗效观察

摘　　要：目的观察比较胃镜下微波烧灼联合局部化疗治疗晚期食管癌和单纯胃镜下局部化疗治疗晚期食管癌的疗效。方法86例晚期食管癌患者分成治疗组(44例)和对照组(42例),治疗组给予微波烧灼联合局部注射化疗药物治疗,对照组给予局部注射化疗药物治疗。治疗过程中观察患者近期疗效、生存期、转移情况、不良反应和并发症。结果治疗组和对照组近期疗效食管梗阻缓解率分别为93.1%、59.5%;治疗组6、12、24个月生成率分别为97.7%、86.4%、65.9%。对照组6、12、24个月生成率分别为92.9%、40.5%、0。两组食管梗阻缓解率、24个月生成率差异有显著性。两组间转移病灶及不良反应差异无显著性。结论胃镜下微波烧灼联合局部注射化疗药物治疗晚期食管癌,疗效较明显,可作为姑息治疗晚期食管癌的有效手段之一。[Objective] To compare the curative effect of gastroscopic microwave cauterization combined with partial chemotherapy with that of simply partial chemotherapy under gastroscope in advanced esophageal carcinoma. [Methods] 86 patients with advanced esophageal carcinomawere divided into treatment group （n = 44） and control group （n = 42）. The treatment group was given microwave cauterization combined with partial chemotherapy under gastroscope. The control group was simply given partial chemotherapy under gastroscope. In the course of treatment, near future curative effect, survival time, metastasis as well as adverse reaction and complication were observed. [Results] The thoracic choke remission rates of near future curative effect in treatment group and control group were 93.1%and 59.5%, respectively. The 6, 12, 24 month survival rates of treatment group were 97.7%, 86.4% and 65.9%, respectively. The 6, 12, 24 month survival rates of control group were 92.9%, 40.5% and 0%, respectively. The thoracic choke remission rate of near future curative effectand 24 month survival rate in treatment group had remarkable difference from those of control group. During treatment both groups had no significant difference in metastatic lesions and adverse reaction. [ Conclusion] For patients with advanced esophageal carcinoma microwave cauterization combined with partial chemotherapy under gastroscope is a better choice of treatment with obvious effect.
