糖尿病患者早期肾损伤时血清铁蛋白、β_2-m和尿微量蛋白水平的诊断价值

摘　　要：目的:为了研究糖尿病(DM)患者早期肾损伤时,血清铁蛋白、β2-m和尿微量蛋白水平诊断价值。方法:放射免疫分析了185例DM和60例正常对照组血清β2-m和尿中的Alb、IgG和β2-m水平,化学发光免疫分析了血清中的铁蛋白水平。结果:185例DM患者血清铁蛋白、β2-m和尿中的Alb,IgG和β2-m水平明显高于60例正常对照组。其中尿AlbP<0.01,尿IgG和β2-mP<0.001;而血清铁蛋白P<0.001,β2-mP<0.01。结论:DM患者血清和尿中的Alb、IgG和β2-m和铁蛋白的测定可以早期诊断肾损伤,从而避免DN的发生。Objective To study the diagnostic value of determination of urinary excretion of Alb, IgG, β2 - m and serum ferritin, β2 - m levels in patients with diabetes mellitus （DM） and early renal function imparment. Methods The urinary excretion of Alb, IgG and β2 - m （wlth RIA） and the levels of serum ferritin （with CLIA） and β2 - m were determined in 185 patients with DM and early renal fanction impairment but no frank nephropathy and 60 controls. Results The urinary excretion of Alb, IgG, β2 - m and serum ferritin, β2 - m levels in the patients were significantly higher than those in controls （P of Alb 〈0.01, P of IgG 〈0. 001 and P of β2-m〈0.001 in urine, P of ferritin 〈 0. 001 and P of β2-m〈0.01 in serum）. Conclusion The determination of levels of urinary excretion of Alb, IgG, β2 - m and ferritin, β2 - m in serum were helpful in diabetic patients for dagnosis of early renal function impairment before the development of frank nephropathy.
