摘 要:目的通过对人离体前磨牙复合树脂充填体的边缘微渗漏研究,探讨在粘结过程中唾液污染对牙齿和光固化型复合树脂之间密合度的影响。方法将88颗人离体前磨牙备洞后用光固化复合树脂充填,根据充填过程中是否受唾液污染及污染后不同的处理方法分为4组,A组未受唾液污染;B组处理后受唾液污染并立即清水冲洗干净再涂粘结剂;C组处理后受唾液污染仅以气枪吹干再涂粘结剂;D组处理粘结后受唾液污染。用染色法对4组充填后的牙齿进行染色,剖开后测量充填体边缘染色线长度,并进行统计学分析。结果①边缘微渗漏深度测量,A组染色线为0·038±0·08mm,B组为0·126±0·19mm、C组为1·471±0·77mm、D组为1·761±0·46mm,B、C、D组充填体边缘微渗漏均高于A组,分别与A组比较,经F检验,B组与A组相比无显著差异(P>0·05),C、D组与A组之间有显著性差异(P<0·05)。②边缘微渗漏分级法统计,经秩和检验,B组与A组差异无统计学意义(P>0·05);C、D组与A组差异有统计学意义(P<0·05)。结论在窝洞涂粘结剂之前受唾液污染并用水冲洗对自酸蚀粘结效果影响不明显,如果处理后受唾液污染仅以气枪吹干再涂粘结剂或者在粘结后受唾液污染则对粘结效果有明显影响。Objective To investigate the effect of saliva contamination on the adaptation between tooth structure and light-cure resin composite. Methods A total of 88 human premolars were prepared in vitro and grouped into 4 categories: A. without saliva contamination; B. saliva contamination after itching and instant water flushing; C. saliva contamination after itching and instant air dry; D. saliva contamination after itching and adhesive application. All the teeth were filled and dried. The colored lines were measured after the teeth were sectioned in the center of the restoration, and the marginal microleakage was observed. Results The marginal microleakage of groups B, C and D was compared respectively with group A. The difference in marginal microleakage was not significant between group A (colored line: 0. 038 ±0. 089 mm)and group B(0. 126 ±0. 19 mm), but significant between group A and group C( 1. 471 ±0. 77 mm) and between group A and group D(1.761 ±0.46 mm). Conclusion Saliva contamination followed by water flushing instantly before adhensive application did not affect adhesion results of serf-itching sealant. Nevertheless the outcome could be affected if the adhesive agent was applied after the cavity was saliva-contaminated and only air dried or if the cavity was saliva-contaminated after the adhesion.
