摘  要:目的:探讨降低原发性肝癌切除术后局部复发,提高肝癌病人长期生存率的方法。方法:300例肝癌病人,按随机数字表分为5-FU缓释微粒植入组和对照组。植入组在肝癌切除术中将缓释5-FU微粒600mg植入肝创面。全部病人术后均行定期随访。结果:植入组术后2、3年肿瘤复发率分别为14.0%和23.3%,对照组为23.3%和34.7%(P<0.05);植入组术后4、5年肿瘤复发率为32.0%和43.3%,对照组为47.3%和59.3%(P<0.01),二者比较有统计学意义。结论:肝癌切除中肝创面5-FU缓释微粒植入能有效地降低术后局部复发率,提高治疗效果,并延长肝癌病人的生存期。Objective To evaluate the efficacy of intraoperative implantation of sustained-releasing 5-FU microparticles in preventing local recurrence in hepatocellular carcinoma(HCC) after hepatectomy. Methods A total of 300 HCC patients underwent hepatectomy from Jan. 2003 to Jul. 2004. The whole series was randomly divided into the control group and the 5-FU microparticals implantation group. In the control group, 150 patients were treated by hepatectomy alone, whereas in the implantation group, 150 patients were treated with hepatectomy plus implanting sustained-releasing 5-FU microparticles on the wound surface. All patients were followed-up regularly. Results The 2-, 3-, 4- and 5-year recurrent rates were 14.0%,23.3%,32.0% and 43.3% in the implantation group, and they were 23.3%,34.7% ,47.3% and 59.3% in the control group, with a difference in the 2-,3-year recurrent rates (0.01〈P〈0.05)and significant difference in the 4-,5-year recurrent rates(P〈0.01 ). There was a significant difference in the survival rate, too(P〈0.01 ). Conclusions Intraoperative implantation of sustained-releasing 5-FU microparticles on the wound surface is an effective measure to prevent local recurrence and to prolong postoperative survival in HCC patients after hepatectomy.
