MRI对中子刀(^(252)锎)放射治疗宫颈癌的疗效评价

摘　　要：目的探讨MRI对宫颈癌中子刀(252锎)放射治疗疗效评估的价值。方法50例经病理证实的宫颈癌患者在中子刀(252锎)放射治疗前及治疗后6天内、2～3个月、6个月、1～2年不同时间点行2～6次不等的MRI扫描。每次MRI扫描均用相同的扫描方法,以相同序列正常子宫肌层信号为参照。在MRI图像上观察肿瘤中子刀(252锎)放射治疗前后的大小和信号改变。结果放疗结束时,局部控制率100%,38例肿瘤消失,12例肿瘤明显缩小,缩小率>85%。2～3个月至2年内不同时间点MRI检查,48例肿瘤消失,12例肿瘤明显缩小较放疗结束时显缩小。肿瘤消失,T2WI及STIR无异常信号改变,T1WI增强扫描,病变区无强化区或轻微强化。结论MRI是评价宫颈癌中子刀(252锎)放射治疗疗效的最佳检查手段,放疗结束时肿瘤缩小程度、T2WI上的信号改变及T1WI增强扫描瘤内有无强化区是预测治疗效果的观测指标,并且放射治疗后2～3个月是MRI预测与评价宫颈癌中子刀(252锎)放射治疗疗效的最佳时间点。Objective To discuss the value of neatron- knife radiation in the evaluation of treatment of cervical cancer with MRI. Methods MRI manifestations obtained in 2- 6 times at different points such as before or after theraphy 6 days, 2-3 months, 6 months, 1-2 years were compared in 50 patients who are confirmed pathologically with cervical carcinoma. Every time, MRI scans applyed the identical method, having the sign of regular array womb muscle as the referrence.The turnout size and the signal changing before and after Neutron-knife radiation therapy on MRI were observed. Results At the end of radiation, rate of part control was 100%. Among all cases, tumour were disappeared in 38 patients, tumors were decreased obviously in 12 patients, the rate of tumors shrinking was greater than 85%, Among MRI scanrfing the tumors were disappeared in 48 patients, 12 patients tumors were decreased obviously when radiation therapy was completed after 2-3 months and 2 years. No signal changed on T2Wl and STIR MR image, and slight or no enhancement on T1WI images. Conclusion MRI is the best examining method in the evaluation of cervical carcinoma response to neutron-knife radiotion therapy. The degree tumour reduces,the chang of T2WI signal, and whether there is a strengthen area in tumour are the observation targets for forecasting therapy effect. After GD-DTPA. Radiation therapy 2-3 months after is the best time to estimate the curative effect of neutron knife radioation therapy with MRI.
