儿童偏头痛在视频脑电图、经颅多普勒及脑干听觉诱发电位的鉴别诊断的临床研究

摘　　要：目的探讨儿童偏头痛在视频脑电图(VEEG)及经颅多普勒超声(TCD)的鉴别诊断及不同治疗。方法选取120例偏头痛患儿同时进行发作期和发作间期VEEG、TCD检查,并进行对照。结果120例偏头痛患儿中,VEEG表现为:波幅电压偏高,局灶性高波幅θ波增多,阵发性短程或中程至高幅的慢波活动为主,及少量间疒样波出现。TCD表现为血流速度快,血管痉挛,并出现湍流等。脑干听觉诱发电位(BAEP)表现为:Ⅰ、Ⅲ、Ⅴ波消失或分化不良,各波潜伏期(PL)及峰间期(IPL)延长。结论在儿童偏头痛患者中,其发作期或发作间期同时检查,能提高诊断率,对治疗有指导意义,减少患儿疾苦。Objective To evaluate the practical value of combined video electroencephalography （VEEG） and trans - cranial Doppler （TCD） for differential diagnosis of pediatric migraine. Methods 120 children patients with migraine were monitored routinely by VEEG and TCD. Results The VEEG in migraine patients displayed elevated wave amplitude and voltage, focal increase of higher θ wave and some epileptiform discharges ; TCD displayed the blood flow was increased and there were vasospasm and swift currents. Brainstem auditory evoked potential in selected cases mainly showed disappearance of I, Ill and V waves or prorongation of PL and IPL. Conelusfons In pediatric patieuts with migraine, combination of VEEG and TCD can help us make diagnosis and differential diagnosis and decide the treatment.
