Cyr61在子宫内膜癌中的表达及意义

摘　　要：目的:研究Cyr61(Cysteine-rich 61)基因在子宫内膜癌及癌前病变中的表达,与子宫内膜癌发生、发展及预后的关系。方法:采用RT-PCR方法和免疫组化S-P法检测21例正常增生期子宫内膜、27例子宫内膜不典型增生及46例子宫内膜癌组织中Cyr 61的表达情况,观察结果并进行统计学处理。结果:免疫组化S-P法结果显示Cyr 61在增生子宫内膜表达最多,其阳性表达率为90.47%(19/21);不典型增生组织中阳性表达率为66.67%(18/27);子宫内膜癌组织中阳性表达率为23.91%(11/46);不典型增生组显著低于增生子宫内膜,子宫内膜癌组显著低于不典型增生组,3组比较差异有统计学意义(P<0.05)。RT-PCR结果与免疫组化法测定的结果相一致,Cyr 61mRNA的表达在增生期子宫内膜、子宫内膜不典型增生、子宫内膜癌患者组织中逐渐降低,统计学分析显示,各组间差异有统计学意义(P<0.05);Cyr 61在子宫内膜癌中表达下降与组织学分级、手术-病理分期显著相关(P<0.05),但与肌层浸润深度、淋巴结转移无关(P>0.05)。结论:Cyr 61在子宫内膜癌和不典型增生中有不同程度的丢失,Cyr 61的低表达促进了肿瘤的发生,检测Cyr 61可成为子宫内膜癌早期诊断及预测癌前病变癌变风险重要指标之一。Objective： To investigate the expression and clinical significance of Cyr 61 gene in endometrial carcinoma as well as between their oncogene expression, development and prognosis of endometrial carcinoma.Methods： Cyr 61 gene was measured by real- time quantitative reverse transcription- polymerase chain reaction （RT- PCR） and immunohistechemical technique （S -P method） in 21 cases of proliferate endometrial and 27 cases of atypical hyperplasia and 46 cases of endometrial carcinoma. Results： The positive - expression rate of Cyr 61 in proliferate endometrial was 90. 47% （ 19/21 ） . There were 66.67% （18/27） and 23.91% （11/46） in atypical hyperplasia and endometrial carcinoma respectively. The positive - expression rate of Cyr 61 in endometrial carcinoma was lower than that in proliferate endometrial and atypical hyperplasia. There were statistic differences among three groups （P 〈 0. 05） . The result was consistent with the immunohistoche mical staining. The expression of Cyr 61 gene Myma was down - regulated in primary endometrial carcinoma patients compared the matched tissues. Statistical analysis revealed that the difference between expression of cancerous tissues and the matched tissues was significant （P 〈 0. 05） . Furthermore, the expression rate of Cyr 61 in endometrial carcinoma was associated with histologic grade and surgical - histological stage （ P 〈 0.05 ） . But it was not associated with the depth of symmetrical tumor invasion and lymphonode metastasis. There were statistic differences （P 〉 0. 05 ） . Conclusion： Cyr 61 gene silencing in endometrial adenocareino ma and atypical hyperplasia. The lower levels of Cyr 61 can promotes endometrial cancer cell growth. Cyr 61 may be as one of the significance indicatory in predicting the risk of carcinogenesis from precancerouslesions.
