摘 要:目的观察缺血后处理对缺血再灌注心律失常的影响,进一步探讨其可能的机制。方法建立大鼠心肌缺血再灌注模型,设假手术组(n=8)、缺血再灌注组(n=8)及缺血后处理组(n=8)。全程监测心电图,再灌注结束后,应用逆转录-聚合酶链反应(RT-PCR)法半定量检测各组Cx43 mRNA的表达水平,并用室性心律失常评分来评价室性心律失常的发生率。结果RT-PCR显示,缺血再灌注组与假手术组比较,Cx43 mRNA的表达水平显著降低(P<0.01),缺血后处理组与缺血再灌注组比较,Cx43 mRNA的表达水平显著增高(P<0.01);监测心电图显示,与假手术组相比,缺血再灌注组的室性心律失常评分显著升高(P<0.01),与缺血再灌注组相比,缺血后处理组的室性心律失常评分显著降低(P<0.01)。结论缺血后处理能抑制由缺血再灌注导致的心室肌Cx43 mRNA降解,从而减少再灌注心律失常的发生。Objective To observe the effects of postconditioning on ischemia-reperfusion arrhythmias in rats,and to explore its possible mechanism. Methods Twenty-four male Wistar rats were randomly divided into 3 groups:sham operation group(n=8),ischemia-reperfusion group(n=8),ischemic postconditioning group(n=8).The myocardial ischemia-reperfusion model was established by ligation of the left anterior descending coronary artery for 30 min,followed by 120 min reperfusion.ECG was monitored during the entire process.The expression of Cx43 mRNA was measured by reverse transcription-PCR,and ventricular arrhythmias was detected with ventricular arrhythmia score(VAS). Results Compared with sham operation group,Cx43 mRNA expression level was signi- ficantly lower in ischemia-reperfusion group(P〈0.01).Compared with ischemia-reperfusion group,Cx43 mRNA expression significantly increased in ischemic postconditioning group(P〈0.01).Compared with sham operation group,ventricular arrhythmias score significantly increased in ischemia-reperfusion group(P〈0.01).Compared with ischemia-reperfusion group,ventricular arrhythmias score decreased significantly in ischemic postconditioning group(P〈0.01). Conclusion Postconditioning could reduce the occurrence of reperfusion arrhythmias by inhibiting degradation of Cx43 mRNA caused by ischemia-reperfusion in ventricular myocytes.
