摘 要:目的探讨脆性组氨酸三联体(FHIT)、细胞外基质金属蛋白酶诱导因子CD147及微血管密度(MVD)表达与垂体腺瘤激素分泌和复发性的关系。方法通过免疫组化S-P法检测FHIT、CD147和MVD(用CD34标记)在63例垂体腺瘤组织中的表达,并结合临床影像学资料分析它们的表达与肿瘤复发的关系。结果分泌型垂体腺瘤(50例)与非分泌型垂体腺瘤(13例)相比,FHIT、CD147及MVD的表达无统计学差异(P>0.05);术后或经综合治疗后复发性垂体腺瘤组织中CD147阳性表达率(100%,6/6)高于无复发者(84.2%,48/57;P<0.05),而FHIT的阳性表达率(16.7%,1/6)低于无复发者(75.4%,43/57;P<0.05),MVD表达水平(59.4333±9.8569)高于无复发者(30.6947±16.2573,P<0.01)。结论FHIT、CD147及MVD的表达与垂体腺瘤激素分泌无关;FHIT的低表达和CD147的高表达可能与肿瘤的复发行为有关;血管形成可能增加了细胞存活能力,高增殖状态对肿瘤细胞抑制降低,易导致肿瘤复发。Objective To study the expressions of fragile histidine thriad (FHIT), CD147 and microvessel density (MVD) in pituitary adenomas and their relationship with tumor recurrence and hormone secretion. Methods The expressions of FHIT, CD147 and MVD (marked by CD34)in 6 cases of recurrent pituitary adenomas and 57 cases of nonrecurrent pituitary adenomas were determined by immunohistochemical technique. Of 63 cases of pituitary adenomas, 50 were secreting type one and 13 nonsecreting type one. The relationship between the expressions of FHIT, CD147 and MVD and tumor recurrence behavior was analyzed based on the clinical and image data. Results There were insignificant differences in the expressions of FHIT, MVD and CD147 between the secreting type pituitary adenomas and nonsecreting type pituitary adenomas (P〉0.05). CD147 and MVD expressions in the recurrence group was significantly higher than those in the nonrecurrence group (P〈0.01), but FHIT expression in the recurrence group was significantly lower than that in the nonrecurrence group (P〈0.05). Conclusions The expressions of FHIT, CD147and MVD are not related to the hormone secretion by pituitary adenomas. Lower expression of FHIT and higher expression of CD147 in pituitary adenomas may be related to the tumor recurrence. Angiogenesis may be linked to the ability of cells to survive. It is suggested that high proliferation behavior and decreased inhibition of tumor cells may induce tumor recurrence.
