血管化腓骨移植同期种植体植入修复下颌骨缺损的动物试验

摘　　要：目的:研究血管化腓骨瓣游离移植钛网塑形同期种植体植入修复狗下颌骨缺损的效果,为临床应用提供依据。方法:对4例狗切除单侧下颌骨体,以自体腓骨瓣血管吻合游离移植修复下颌骨缺损,以微钛板内固定,钛网塑形人工骨粉植入恢复下颌骨形态及完整性,同期用人工种植体各一枚植入移植腓骨处,定期进行血液指标检测,X线检查及动物处死后组织学检查。结果:X线、骨密度测定、血清碱性磷酸酶(ALP)、血清骨钙素(BGP)、下颌运动功能检测、扭转、剪切力、压力、弯曲试验结果显示,植入物愈合正常,下颌外形恢复满意。结论:血管化腓骨瓣游离移植钛网塑形同期种植体植入修复狗下颌骨缺损是可行的,重建下颌骨的功能和外形良好。Objective：To study the therapeutic effect of reshaping and reconstruction the mandibula defect in dogs by means of vascularized fibular free flaps and the implants immediately placed into the fibula,in hopes of providing clinical evidence.Methods： mandibula is removed first followed by transplanting fibula which is fixed by mini-plate,reshaped,titanium net is filled with artificial calcium sulfate.Four implants are planted respectively in the fibula in the four dogs. Periodic examinations such as blood testing,x-rays and tissue studies and biomechanic index from killed dogs are done.Results：The x-rays studies, the level of ALP and BGP bone,miner- al density and biomechanic index,all these studies show that the transplanted heal well and the shape of mandibula recovers satisfactorily.Conclusion：The above methods used in rebuilding the mandibula in dogs is practicable in achieving the normal function and shape of the mandibula.
