山东省县乡两级医院护士工作压力源研究

摘　　要：目的了解山东省县乡两级医院护士主要工作压力源,为改善护士压力状况,提高县乡两级医院护理质量提供依据。方法采用分层、随机、整群抽样法,应用护士工作压力源量表对480名山东省县乡两级医院护士进行调查。结果山东省县乡两级医院护士的工作压力源总分为2.35±0.43,5个维度中时间分配及工作量方面评分最高(2.77±0.68);35个条目中评分最高的是工作量太大(2.95±0.85);不同婚姻状况、地区、科室、班次护士压力源评分比较,差异有显著性意义(P<0.05,P<0.01);县级医院护士压力源评分显著高于乡级医院护士(均P<0.01)。结论山东省县乡两级医院护士工作压力适中,县级护士工作压力高于乡镇护士;工作量大是最主要应激源。政府部门应有针对性地加强对基层医院护士压力状况的干预,减轻其压力程度,提高其心理健康水平,从而提高护理质量。Objective To survey the work-related stressors of nurses in hospitals of both county and country levels in Shandong province and to advance appropriate policies to improve the stress situation of nurses in the rural nursing service. Methods By stratified, random, clustery sampling, a survey consisting of the Nursing Stress Scale was administrated to 480 nurses both in the county and country hospitals. Results The level of work-related stress of nurses in the above two levels of hospitals was moderate with a mean score of 2.35±0.43. Affaires of the arrangement of time and the working volume were proved to be the most stressful events for nurses with a mean score of 2.77 ±0. 68. The highest score of the 35 items was the high working volume. The scores of stress could be affected by the difference of marriage, area, department and working style （P〈0.05,P〈0.01）. Nurses in the county hospitals were found to experience higher degree of work-related stress compared with their peers in the country hospitals（P〈0.01 for all）. Conclusion The overall level of stress of nurses in hospitals of both county and country levels is moderate, nurses in the county hospital experience higher degree of wore-related stress, high working volume is the main stressor. It is of great urgency to relieve the stress level for the rural nurses through more attention and intervention and hence to approve the nursing care.
