枕部开颅切除松果体区肿瘤48例临床分析

摘　　要：目的探讨枕部的开颅术切除松果体区肿瘤的手术方法和治疗的经验。方法回顾性分析48例经枕部开颅手术切除松果体区肿瘤患者的临床资料及术后早期随访结果。结果48例患者均经小脑幕入路,其中肿瘤全切除44例,次全切除3例,部分切除1例。术后死亡1例,失语、轻偏瘫1例,视野缺损2例。8例出现早期或迟发性脑积水。结论枕部开颅经小脑幕入路切除松果体区肿瘤,效果可靠,值得推广,但要重视脑积水等并发症。Objective To approach the surgical methods and therapeutic experience of cutting tumors of pineal region via occipital craniotomy. Methods To analyze retrospectively the clinical data of 48 patients with tumors of pineal region accepted occipital craniotomy and early follow up result. Results All 48 patients accepted transtentorial approach including 44 tumor total resection, 3 secondary total resection and 1 partial resection. One was dead postoperatively, 1 anepia and hemiparesis and 2 defected of field vision. 8 showed early or tardive dropsy of brain. Conclusion The effect of cutting tumors of pineal region using occipital craniotomy via transtentorial approach is reliable and it deserves spreading,while complication such as dropsy of brain should he paid attention to.
