光敏灭菌辅助龈下刮治后不同深度牙周袋的恢复率比较研究

摘　　要：目的评价PERIOWAVETM光敏灭菌法治疗慢性牙周炎后≥4mm牙周袋的恢复效果。方法经伦理委员会同意后,选择45例患者(每位患者口腔内至少有4个以上≥5mm的牙周袋并分布在2个以上口腔区域内),在接受基础检查以及根面平整术(SRP)治疗后,随机分入3组,A组在SRP之后接受1次PERIOWAVETM光敏灭菌治疗,B组在SRP后及6周后各接受1次PERIOWAVETM治疗,而C组仅接受SRP治疗。在基础检查及12周后测量牙周袋深度。结果在基础检查中共有1851个≥5mm牙周袋,经过治疗后,C组有212(36.7%)个牙周袋≤3mm,A组有160(62.5%)个牙周袋≤3mm,而B组有187(84.6%)个牙周袋≤3mm;705个4mm的牙周袋,治疗后PD≤3mm的C组147(66.5%)个,A组160(62.5%)个,B组187(84.6%)个。经两样本二项分布检验,对于基础检查中为测量为4mm及≥5mm牙周袋,B组的恢复均明显好于A组及C组(P<0.0001)。结论在SRP基础上加以2次光敏灭菌治疗牙周炎对于浅(4mm)及深(≥5mm)牙周袋深度的恢复均明显大于单独使用SRP。Objective To determine full recovery rates （improvement to ≤3 mm） of ≥4 mm periodontal pockets treated with a new photodynamic treatment in a randomized and blinded clinical trial. Methods With Ethics Committee approval, 45 subjects [each with ≥4 sites of periodontal pocket depth （PD） ≥5 mm in ≥2 quadrants] were enrolled in the study. After baseline examination, all subjects received scaling and root planing （SRP） and were then randomly assigned to one of three groups. Group A received one Periowave photodynamic treatment （Ondine Biopharma Corp., Vancouver, BC） after SRP, while Group B received one Periowave treatment after SRP and then another 6 weeks later. Group C received SRP alone to serve as the control. PD was measured at baseline and 12 weeks. Results Totally, 1,851 sites with PD ≥5mm were present at baseline. After treatment, 212 （36.7%） sites in Group C showed a PD ≤3 mm. In Group A, 214 （37.2%） sites fully recovered to a PD ≤3 mm. In Group B, 389 （55.7%） sites showed full recovery to a PD ≤3 mm. For 4 mm PD, 705 sites were present at baseline. Post-treatment examination found that 147 （66.5%） sites in Group C recovered to a PD ≤3 mm. In Group A, 160 （62. 5%） sites recovered to a PD of ≤3 mm, while in Group B, 187 （84.6%） sites fully recovered to a PD of ≤3 mm. Data analysis using the normal approximation to a twosample binomial test demonstrated that, for both PD sites at 4 mm and ≥5 mm at baseline, the recovery rates in Group B were significantly higher （ P 〈 0. 0001 ） than those in Groups A and C. Conclusion Under conditions of this study, two photodynamic treatments after SRP significantly increased full recovery rates in both shallow （4 mm） and deep （ ≥5 mm） periodontal pockets, compared with SRP alone for periodontal disease therapy.
