中高度近视、远视性弱视综合治疗的临床分析

摘　　要：目的观察并了解中高度近视、远视性弱视儿童的综合弱视治疗特点及疗效。方法对78例中高度近视及远视性的弱视儿童,先散瞳验光配镜,然后用已证明疗效稳定的统一品牌的弱视治疗仪(包括红闪、光刷、后像)进行集体、集中治疗,回家完成精细作业(包括描图、剪纸、穿珠子等),同时根据两眼弱视程度及患儿年龄选择不同比例遮盖一侧眼。定期复查调整镜片和治疗方案。结果中高度近视、远视两组弱视儿童综合弱视治疗痊愈率分别为80%及93%。轻、中、重度痊愈率分别为96.6%、88.6%、64.3%,有明显差异。中高度近视性弱视学龄前、学龄期痊愈率分别为72%、88%。中高度远视≤6岁、≤8岁、≤10岁弱视痊愈率分别为100%、92%、60%。结论中高度远视性弱视疗效优于近视性弱视,两者疗效(痊愈率)有显著差异(P<0.05);中高度近视及远视弱视程度越轻,效果越好(P<0.05);学龄前中高度近视弱视程度重,痊愈率低于学龄期;中高度远视初诊年龄越小,弱视有效率及痊愈率越高,疗效越好(P<0.05)。Objective To study the character and effect of combined therapy in children with different ametropic amblyopia by observing the combined therapy in 35 cases （70 eyes ） of medium and severe myopic amblyopia and 43 cases （68 eyes） of medium and severe hyperopic amblyopia. Methods All cases with amblyopia （156 eyes） were treated with combined therapy, including lens therapy, occlusion and active therapy, Cam vision stiumlator, eye-hand coordinated games et al. Review at monthly appolintments. Results The cure rate in medium and severe myopic amblyopia was 80% , while that in medium and severe hyperopic amblyopia was 93%. The cure rate in light, medium and severe amblyopia were 96. 6% , 88.6% and 64. 3%. The cure rate in medium and severe myopic amblyopia in the pre-school and that in the school were 72% and 88%. The cure rate in medium and severe hyperopic amblyopia of ≤6 years,≤8 years and ≤ 10 years were 100% , 92% and 60%. Conclusion The curative effect of combined therapy in children with ametropic amblyopia is related to ametropia type, degree and age. The cure rate in hyperopic amblyopia is better than that in myopic amblyopia （ P 〈 0. 05 ） . The lighter the amblyopia degree is, the better the effect is （P 〈 0. 05） ; The younger the child is, the better the effect is （ P 〈0. 05） . But the cute rate in myopic amblyopia in the school children is better than that in the pre-shcool ones.
